You have new benefits starting January 1, 2024!

Due to a recent change in contract, employees now have FREE (single) employer-paid health insurance with UNITE HERE HEALTH’s Food Service Plan. You’ll be automatically enrolled in single coverage, which includes:

• Gold Plus PPO Medical/Rx
• Dental PPO
• Vision
• NEW! Life, AD&D, and Short-term disability (employees only)

ACTION REQUIRED! If you want to add dependents (this costs extra and will be deducted from your pay) or waive your single coverage benefit (details below): Go online or call: December 1 – 8, 2023

What you’ll need:

Marriage certificate (for a spouse)

Birth certificate (for a child)

Your last day to turn in certificates is December 8.

To waive coverage:

• You MUST provide written proof that your other insurance provider does not allow you to have double coverage.
