On behalf of the Accountable Health Partners Quality Team, below are the AHP year-to-date quality scorecards for 2024. These scorecards represent calendar year performance from January 1st, 2024 through June 10th, 2024, so we anticipate the numbers will continue to rise as additional patients are seen.
As previously communicated, this year’s performance will be measured at the network level (for all AHP primary care providers combined), and for our Excellus patient population only. This change was made for 2024 to offer consistency as we transition our data aggregation platform to Health Catalyst. We have attached the Excellus Commercial and Medicare scorecards for the entire AHP Network. Medicaid results are pending and will be shared when they are available. This structure will remain in place through the end of 2024, and we will reevaluate for 2025 based on the data available.
