To support the implementation the Enhanced Care Management (ECM) benefit and Community Supports services under California’s Medi-Cal transformation known as CalAIM, the Department of Health Care Services launched the Providing Access and Transforming Health (PATH) Initiative which provides funding to address gaps in local organizational capacity and infrastructure throughout the State, over a five-year period (2022 – 2027).
Chapman Consulting is proud to support the PATH effort as facilitators for the Collaborative Planning and Implementation Initiative (CPI) workgroups in San Francisco and Santa Clara counties. The CPI workgroups bring together community-based organizations, Federally Qualified Health Centers, Managed Care Plans, state and local government entities, and other health care providers to work together on identifying and resolving local implementation issues, and disseminating best practices for delivering ECM and Community Supports under CalAIM.
Through intentional collaborative efforts, stakeholders have successfully identified strategies and unified processes to create a more integrated, patient-centered healthcare system that addresses the complex needs of San Francisco and Santa Clara counties’ underserved populations and promotes health equity. The Chapman team has also developed a suite of tools and resources to help local providers navigate the landscape, including an ECM and Community Supports Access Guide, Provider Toolkits, and updated Provider Rosters.
