Third Space Endoscopy Program Transforms Gastrointestinal Care in Harris County
Third Space Endoscopy, also known as submucosal endoscopy, is based on the principle of tunneling to the deeper layers of the gastrointestinal tract without compromising mucosal integrity. This is performed to remove deeper lesions or treat conditions requiring us to go deeper into the lining of the digestive system (for example, the muscle layer). These highly specialized complex procedures are minimally invasive and offered as an alternative to traditional surgical procedures for these conditions. Most common procedures include per-oral endoscopic myotomy (POEM) for the treatment of achalasia, gastric POEM for the treatment of medically refractory gastroparesis, and Endoscopic submucosal dissection (ESD) for R0 resection of early esophageal, gastric and colon cancers.
Vaibhav Wadhwa, MD, Director of Interventional Gastroenterology (iGUT) at LBJ Hospital, and Pritesh Mutha, MD, Director of Third Space Endoscopy for UTHealth Houston started this program with the sole purpose of helping the underserved Harris County population ailing with these conditions. These patients have nowhere to go and cannot afford to get these procedures done at other health systems in Houston. The program was started early this year, and the team has completed 15 cases, including ten esophageal POEMs, two gastric POEMs, one rectal ESD, and two gastric ESDs. Almost all patients who underwent POEM were discharged the next day, with those undergoing ESD discharged the same day. As noted on follow-up appointments, all patients have had excellent responses to their symptoms.
Currently, the procedures are performed once a month, with plans to increase it to twice a month based on volume and referrals.
The procedures performed at LBJ at the moment:
- POEM for Achalasia
- G-POEM for refractory gastroparesis
- ESD for early gastric cancer and early colon cancer
For more information about the program and how to refer your patients directly, please email Vaibhav.wadhwa@uth.tmc.edu.
Third Space Endoscopy Program is part of the comprehensive interventional gastroenterology services provided at LBJ by Vaibhav Wadhwa, MD, and Mairin Joseph-Talreja, MD. The iGUT team at LBJ performs the highest volume of complex interventional gastroenterology procedures in the Harris County system.

At the Bedside: Pancreatic cancer patient ‘back on track’ after UTHealth Houston clinical trial therapy shrinks tumor
|
Demetrios Stroubakis, 55, thought he was having a gallbladder attack when he arrived at the emergency room in August 2021 with jaundice. Instead, he was shocked to be diagnosed with pancreatic cancer.
“I was floored. I consider myself to be relatively healthy, so I wasn’t expecting it,” Stroubakis said. “I was fortunate my wife Shannon recognized the jaundice conditions. She rushed me to the emergency room and after several scans and a tissue biopsy over the course of few days, I got the bad news.”
The pancreas produces enzymes to help digest food and makes the hormone insulin, which controls blood sugar. About 64,000 people will be diagnosed with pancreatic cancer this year, and nearly 51,000 will die from it. Pancreatic cancer is the third leading cause of cancer death in the U.S. and has a five-year survival rate of 12.5%.
