HPC was founded by the community, for the community, and we continue our work today thanks to your generous support and partnership. Please take a moment to read our 30th Anniversary Report and see the growing list of generous 30th Anniversary Sponsors.

Danielle, a graduate of our REACH Project, set one of her goals to continue her education. She used her financial stipend to help pay for courses at Cape Cod Community College, with the skills she obtained in REACH. We are so proud to share with you Danielle’s recent graduation from 4 C’s with an associate degree in criminal justice.
“I took five classes while working two jobs. The earliest I went to bed at night was 3 am. I was always so tired, but I pushed through and did my best. I just want to say thank you – if I didn’t enroll in the REACH program, it would have been difficult for me."

Maeve Myles, a high school senior, has gone above and beyond in creating her team "HOPE for the HOMELESS." She is raising funds for HPC as a community outreach project for her National Honors Society by encouraging friends and family to support, and participate in, the Walk For Home. Thank you, Maeve!

We're Back!
HPC in Person, in the Community!

Meeting in person with those in our community including our senior, isolated community members, and immigrant neighbors is critical to our intervention and effort to leave no one behind in our mission to promote stability for all who live in our community.
We have just resumed our weekly on-site outreach efforts at the Family Pantry of Cape Cod every Tuesday from 11:00 to 3:00 and will be expanding our various outreach efforts in the weeks ahead.

Let's Talk About Tomorrow

Our next Let's Talk About Tomorrow information session will be presented by the Chatham Council On Aging on Thursday, June 17th at 9:00 AM via Zoom. This information is crucial for older adults who are considering either applying for affordable senior housing or aging in place. To register, call the Chatham COA at (508) 945-5190.

The REACH Project
Charting a Path to Success
Resiliency, Empowerment, Achievement, Community, Hope

HPC is honored to be the recipient of a $25,000 grant in support of The REACH Project from the Cape Cod Healthcare Foundation. Currently, 17 individuals are participating in this four-month project. REACH helps individuals achieve long-lasting self-sufficiency by assisting them in charting their path to success through mindfulness practice, individualized coaching, and goal setting.
