Discovery Session Guide for Live Videoconferencing

The Discovery Session is a 60-minute introductory session designed to help you recruit and enroll participants into your National DPP Lifestyle Change Program.


The Discovery Session facilitator should be someone who knows the program well. It can be the lifestyle coach who will lead the lifestyle change program classes, your program manager, or a past participant.


·      The Discovery Session Provides an overview of the National DPP lifestyle change program.

·      Explains how participants can improve their health by using the lifestyle change program’s proven behavioral strategies.

·      Establishes a social connection with the lifestyle coach and other participants.

·      Helps participants understand their ability to successfully improve their health.

·      Helps participants identify healthy lifestyle skills that can be used right away.

Encourages people who are at risk of developing type 2 diabetes to enroll in the lifestyle change program.

The Using A1C to Meet Program Requirements Webinar provides CDC-recognized organizations delivering the National DPP lifestyle change program an overview on how A1C values can be used to meet program requirements. The webinar details how organizations can use A1C to determine participant eligibility for the program, how they can use it as a key outcome measure to demonstrate type 2 diabetes risk reduction, and what the reporting requirements are for A1C (including the protocol for organizations to record A1C data).

In March 2023, as part of the Medicare Diabetes Prevention Program (MDPP) Enrollment Project, the National Association of Chronic Disease Directors (NACDD) conducted interviews with two experienced MDPP suppliers to understand best practices for building partnerships that result in increased referrals to an MDPP program. The two featured interviewees were selected based on their roles as peer mentors in the MDPP Enrollment Project. 

Based on the interviews, this resource distills best practices for various types of partnerships that MDPP suppliers may consider:

  • Area Agencies on Aging (AAAs)
  • Cable Access Television and News Publications
  • Colleges and Universities
  • Elected Officials
  • Senior Living Facilities
  • Trustees and Board Members
  • Staff, Patients, and Public Health Officials
