Certificate of Immunizations (CIS) - all grades

Immunization records must be up-to-date by the first day of school! The Washington Immunization System makes it easy to auto-fill information: WA DOH.

• Note: Kindergarten and 6th grade students are due for an update to their immunizations.
• Turn in to the school office

Summer Office Hours

• The office is open Monday to Friday 9:00 am to 3:00 pm or by appointment.
• Staff In-service next week means all staff will be out of the office Tuesday, Aug 27th and in meetings Wednesday and Thursday most of the day.
