Those who received their first Covid-19 vaccine on February 25 at the Fremont School vaccine clinic will be receiving their second Covid-19 vaccine on March 18. Below is all the information you will need to be fully prepared for this opportunity. Note that this information is received directly from the pharmacists who have procured this vaccine in order to facilitate the clinic. All the information in this email supersedes any emails that are automated by Jewel OSCO.
PLEASE DO NOT arrive more than 5 minutes early. You will not be admitted until your time slot and you will cause more chaos and longer lines. Simply wait in your vehicle until it is closer to your time. We owe it to those coordinating this for you to be prepared and do all we can to assist in having this run smoothly and efficiently. Please adhere to all that the clinicians are asking of us, listed below:
Our Jewel OSCO pharmacy clinicians who organized this event have, in writing, confirmed that all those receiving DOSE #2 will require:
1) Vaccination card - given to you at the time of your first immunization on February 25.
2) Completed Informed Consent for Immunization form - standard operating procedure for every vaccination received in the state of Illinois. You are giving consent each and every time and by having this completed in advance, you will reduce the wait for yourself and others, avoiding filling it out the day of the event. 3) SHORT sleeved shirt! Much time was wasted in round #1 with those struggling with long sleeves.
4) Your assistive device – walker, wheelchair – if needed. Fremont only has a limited number of these, so if you have one, please bring this along with you for your use.
The time of your next vaccine is at the same time you had your last vaccine, unless you changed it via email using the response form that were emailed to you.
