ASK Day- June 21st

Asking one question might save a child's life.


ASK (Asking Saves Kids) Day serves as a reminder to all caregivers to ask if there is a firearm inside the home where their kids will be playing and if it's stored, locked and unloaded. It's also a reminder for all families to secure their own firearms within their home.

Quality Programs

TCCN Minimum Standard Scorecards

TCCN is committed to Network Quality Improvement and the ability to demonstrate change in practice. As we continue to improve and evolve as a network, the setting of minimum standards is an important step to take in order to ensure compliance and meet Clinical Integration Requirements. 


For this year, we will continue to place a focus on Asthma Action Plan, BMI coding, and depression screening as important measures of focus.


As a reminder, the minimum standards for 2024 are:

 

  • All practices are required to meet the minimum standard of 15% for coding for Asthma Action Plans by 12/31/2024.
  • All practices are required to meet the minimum standard of 55% for coding for BMI by 12/31/2024.
  • All practices are required to meet the minimum standard of 45% for coding for Depression Screening by 12/31/2024.


Although the well child 3-21 metric is not currently a component of our minimum standard program; it is a measure of focus for this year and we are including it so that we can continue to monitor our progress on this important measure as a network.

Exploring New Frontiers: Care Management

Thank you to everyone who attended the virtual Clinical Quality Forum on April 11. This module was the official kick-off for our Care Management program.


If you were not able to attend live, we highly encourage you to watch the recording via this link (https://www.surveymonkey.com/r/DNS67VC) or the QR code below to receive your TCCN participation credit. 


The recording is roughly one hour. If you attended live, there is no further action needed for your practice to receive credit. As a reminder, at least one provider from each practice must participate in one educational event in 2024, and this event fulfills that requirement.
