The purpose of this newsletter is to facilitate the spread of sustainable, promising practices to all dialysis facilities in our service area. Each section listed below coincides with a Centers for Medicare & Medicaid Services (CMS) Quality Improvement Activity (QIA). The interventions are CMS focused for nationwide efforts with these QIAs. The sections contain resources and information that were rated highly effective and beneficial by your facilities. Please read through each section of the newsletter and use the resources with your staff and patients. Please consider reaching out to the quality improvement department.

Community Coalitions

Our community coalitions are made up of several different stakeholders, physicians, clinical staff, and patients that serve as work groups to assist with our Quality Improvement Activities. We will be working with our coalitions to hold discussions, brainstorm ideas, and formulate action plans to assist dialysis facilities in several quality improvement areas. American Kidney Fund (AKF)

Talk to Your Patient Guide: Anemia and Kidney Disease


There is often not enough time in one appointment to cover every topic related to CKD or ESRD in depth. In order to help maximize time and facilitate anemia discussions, AKF launched ACT on Anemia, a patient education campaign designed to increase awareness of anemia as a serious condition associated with kidney disease.

Living Donation Kidney Transplant


Living donation is when a living person donates an organ, or part of an organ, to another person. It is a way to receive a kidney transplant sooner! Living donors don’t need to be related.
