Physical Night

Location: Piedmont High School

Thursday, July 11th at 5:30 p.m.

Students can start lining up at 5 p.m.

Student-Athletes in Grades 7-12

Physicals and online forms must be completed before a student-athlete can participate in a practice or a game for the upcoming season.


All incoming 7th-12th grade students participating in athletics (marching band included) must complete required paperwork and have an annual physical on file prior to participating in practices or games.

Those interested in completing their physical exam through the school must register online. We are able to offer this service at a discounted price of $20. Mercy doctors and nurses will be on-site to complete all the necessary steps to meet these requirements. 

Procedures for Physical Night:

  • Doors will open at 5:30 p.m.
  • Enter through the high school front doors
  • Lines for physicals will form near the gym
  • HS cafeteria will be reserved for parent seating
  • Parents will not be allowed to stand in line with student-athletes
  • A section of the cafeteria will be reserved for filling out paperwork (computers will be provided)
  • Coaches will be available to answer any questions

Do you plan on getting a physical from a family physician?

Before going to the doctor, we ask that you download, print, and complete page one of the “Physical and Parent Consent” form. This is a 2-page form and is located on the “Download and Print” drop-down tab located on the Rank One sign-in page (Directions are below how to access this). A doctor must complete page two.

How to complete Required Paperwork:

In order to complete the required paperwork, please follow the directions below:

  • Go to www.rankonesport.com
  • Click on “Fill Out Electronic Forms” in the top left corner of the screen
  • Choose “OK” as the state from the dropdown box 
  • Scroll down and select “Piedmont Public Schools”
  • Read the directions and proceed with the required paperwork.
  • You can sign in to your parent account using Google, Facebook, or by using your email address if you have previously used Rank One. If not, you will create your own parent account and add each child to your account.
