The new tool, Participant Chart Requirements document, has been developed to assist providers in managing client files. The goal is to use this tool throughout the year in order to keep participant charts up to date and aligned with HUD and CTBOS requirements.
The tool indicates which documents should be in CoC Program Participants' charts for:
- Permanent Supportive Housing
- Raid Rehousing: includes DV Bonus, YHDP RRH and YHDP Diversion/Rapid Exit
- Transitional Housing: Includes YHDP Crisis Housing
- YHDP Youth Navigator
Links to required forms, sample forms and other guidance are also included! Please take advantage of this valuable document which compiles information from a variety of sources into one convenient tool.
