Join the selected Fulcrum Health referring providers to the federally funded study called MN RETAIN and receive patient resources to get them back to work.  MN RETAIN, in collaboration with the State of MN, supported by the Mayo Clinic, is conducting a multidisciplinary, statewide study supporting injured or ill employees helping them to stay at or to return to work.


Why become a referring provider?  

  1. A job is a social determinant of health because it effects the physical, mental, and financial well-being of people.
  2. To improve job retention for MN workers and reduce the likelihood that workers would become disabled. Work with the Dept of Labor and help to minimize the number of people on social security disability.
  3. Chiropractors have the opportunity to be a part of a federally funded statewide study.
  4. It’s easy! All you need to do is connect your patient with a MN RETAIN case manager. Let MN RETAIN do the work!
  5. Receive a $100 gift card for you* and your patient** as a thank you.

ScoreCare Updates Now Available

Fulcrum Health strives to be a leader in maintaining high-quality physical medicine networks and improving the quality of life of our communities by delivering high-value networks.


We are excited to announce additional enhancements to the clinical performance metrics used to determine the tier status of ChiroCare network providers. These additional metrics support providers and clinics in understanding the value of delivering care with the goal of achieving the Triple Aim (patient satisfaction, quality outcomes, affordability)


Provider Tiering assignment

Your 2024 provider tier assignment will now be determined by the sum of average visit utilization plus total services per visit, in addition to the current metrics of the average percent of the 10 and 15 visit cases.


Provider and Clinic performance

Follow your performance by visiting Fulcrum’s Provider Portal to review your scorecard report.


In addition to the provider CMT code selection, x-ray utilization, average percent of the 10 and 15 visit cases, and submission of outcome assessments, you will now be able to view the sum of average visit utilization, and total services per visit. Additionally, new for 2023 all the above metrics will be shown at the clinic level as well as the existing provider level.


Scorecards are only available to providers when meeting the 15 patient minimum requirement during the review period.

Claims Submission Errors

What to do if you make an error on a claim submission:


  • If you make an error on a submission of a claim, and/or receive claim denial notice due to a coding error, you must submit a valid replacement claim in order for the claim to appropriately process.


Tips for submitting a valid replacement claim:

Include the following in Item 22 on CMS 1500 form or in the ANSI 837 CLM REF segment and Qualifier in Loop ID 2300:

  • The original reference/claim#
  • Claim frequency/resubmission code “7”


Note: Failure to follow the above steps, will often result in a “duplicate claim” or “incorrect payor” denial. The claim cannot process accordingly until a valid replacement claim is received.

Correct Email Address? Changes to Clinic and/or Provider Information? Let us Know!

The federal No Surprises Act requires health plans to verify the accuracy of provider contact information every 90 days. Fulcrum will provide more information about this requirement and what it means for you and for Fulcrum in a future communication.


Currently, your provider agreement with Fulcrum requires you to immediately notify Fulcrum of any change in your or your clinic’s demographic information, including, but not limited to name, address, phone, fax, email address, and taxpayer identification number.


Fulcrum’s Provider Portal offers a quick and electronic method for reporting changes.  Simply select the Demographic Updates tab on the home page.
