Social determinants of health (SDOH) are the conditions in which persons live, work, play, and access care (CDC, 2022). SDOH are the complex, integrated, and overlapping social structures, policies, and economic systems that affect health and quality of life outcomes. SDOH can also be a significant cause of health inequities, particularly when populations do not have access to the same resources. Social determinants of health, such as unequal access to health care, stigma, and racism, are underlying factors in health inequities.
The Center for Practice Innovations is pleased to announce a new training module, Foundations of the Social Determinants of Health. This introductory course will:
- Define social determinants of health, social needs, and their impact on individual health.
- Describe how unmet social needs associated with social and structural determinants of health (SSDOH) impact the health outcomes of individuals and communities.
- Explain how healthcare entities can assess and address their patients' unmet social needs.
