Thank you for enrolling in our mailing list for various insurance programs. We want to thank everyone for their ongoing interest and truly appreciate your patience as we continue to work on certain features, even though we would love to have everyone enrolled right away! For now, we are only offering Silver Sneakers and hope to add other memberships as we learn with the community! | |

SilverSneakers® by Tivity Health®, the nation's leading fitness program for older adults, will offer members more opportunities than ever in 2024 to improve their health and well-being through physical activity, social engagement, and mental enrichment. The program was founded in 1992 and is available to more than 18 million Americans through select Medicare Advantage plans, Medicare Supplement carriers and group retiree plans at no additional cost.
Through this collaboration, the New Berlin Activity and Recreation Center will now offer older adults more opportunities to increase their levels of physical activity while motivating them to remain active.
While we are only offering Silver Sneakers, we hope to include other memberships in the future! For now, setting up your Silver Sneakers membership will be by appointment only from 12 pm-2 pm. Appointments will be thirty minutes for any questions or concerns you may have! August 20th was our first day of appointments, which went so well! On behalf of our team, thank you for your kindness and patience as we take the first steps in our enrollment process. We will be adding more appointment dates so please stay in touch! Please keep in mind that we are not holding appointments for RenewActive, if you have United health Care they may have switched your Silver Sneakers to RenewActive, which we are not yet accepting.

Eligibiltiy
There's a Good Chance you’re Eligible! Did you know 1 out of 5 people aged 65 and older are eligible for this fitness benefit? If you're a group retiree or part of a Medicare health plan, you may already have a SilverSneakers membership.

Equipment includes volleyballs, basketballs, pickleballs, pickleball paddles, foam balls, and more!

What do I bring with to my appointment? | |
IMPORTANT APPOINTMENT INFORMATION
1) If available, your 16-digit Tivity Health ID, otherwise called your Silver Sneakers ID. (Please call the service provider on the back of your insurance card to verify OR follow the steps outlined in the graphic).
If you are unable to obtain your 16-digit ID, we will need the following information:
1) Health plan ID and name (this is on your health insurance card)
2) First name, middle initial, last name
3) Date of birth
4) Phone number
5) Email
6) Address
7) Emergency contact information (it is recommended to have your spouse, child, or other trusted adult such as a neighbor)
8) A smile in case we need to take pictures!

Your 16-digit Tivity Health ID is your Silver Sneakers ID, but you still might be wondering how you obtain this! Please use the picture called "How to get your Silver Sneakers ID," which outlines these steps. To find out if you qualify, visit the SilverSneakers website at https://tools.silversneakers.com/Eligibility/CheckEligibility. Once you are enrolled, present your 16-digit code to our front desk staff. We’ll handle everything else! If you do not have your code at your time of visit, no problem! Paper copies are available at the City Hall, Community Center, Library, and the Activity and Recreation Center.
As a reminder, classes will be an additional charge.

When you obtain your Gym/Track Silver Sneakers Pass you have access to our track, wellness mezzanine, and our open gym time. We offer specific open gym times for pickleball, basketball, and volleyball. In addition, we offer times for family open gym and a general open gym time. You are welcome to check out any of our equipment and use the lockers to store bags, clothes, or other personal items.

Amenities
Other amenities do include lockers, bathrooms, a concession stand with food and drinks, and an elevator for your convenience
There is three full size courts, a track, and wellness equipment. (Treadmill not included).
We do not have a pool, showers, vending machines, or locker rooms

1-36 Pick and Choose!
To obtain a locker or equipment, please exchange with front desk your drivers license or comparable item. Locker usage is only for the day that you are here. Please see front desk for more details!
In the event that you misplace your locker key or any form of locker damage occurs you will be assessed a replacement cost. It is highly advised that you always carry your key with you and keep it on the key ring. Do not lend your key for any reason. Do not attach any other items to your key or key ring. If you lose your locker key, immediately notify the Recreation staff and they will assist you in processing the required fees. The key replacement fee is $25.00 and the locker damage fee is $50.00. Please note that these fees are subject to change
