The Middlesex London Ontario Health Team (MLOHT) is seeking an exceptional primary care leader to work effectively with Middlesex London OHT staff, members, and stakeholders in the planning, implementation, and sustainability of the SCOPE model.  The SCOPE "Seamless Care Optimizing the Patient Experience" model aims to improve the overall patient and provider experience within the healthcare system. The model supports and empowers a collaborative network between primary care providers (PCPs), hospital, and community services through a single point of access to a virtual inter-professional care team led by a nurse navigator. The SCOPE model facilitates system navigation and provides primary care with urgent access to specialists, diagnostic testing, and other needed hospital and community services.

INVITATION TO PARTICIPATE IN A SURVEY FOR BUILDING METHAMPHETAMINE PATHWAYS

Caring for people who use or are at risk of using methamphetamines is complex. Many elements of care require a highly-trained specialist and adaptation to the individual's specific needs. However, we also know that there are some standard activities that should be made available to every individual regardless of their circumstances.


In this survey, we are trying to gain consensus around these core activities to help with:  


  • Explaining to individuals using substances and their providers what supports should be made available; and
  • Determining what resources are needed to provide these necessary elements of care for everyone who needs them 


This survey is for anybody who may directly or indirectly interact with people who use substances. Please read every activity carefully and use your experience and best judgement when responding to questions. The goal is to refine the current list to only activities that "should be required for everybody."

NATIONAL DAY FOR TRUTH AND RECONCILIATION

Eric Hendrick, the Middlesex London and Elgin OHT Indigenous Health Improvement Facilitator, has been very busy during his first year in this role!


On September 13th, Eric brought the Operations Teams members from both OHTs together for a “deep dive” into Truth and Reconciliation. We were grateful to listen to and share stories and experiences and, importantly, to think about our personal and collective commitments to reconciliation.

TURNING KNOWLEDGE INTO ACTION: CREATING SAFER SPACES FOR INDIGENOUS PEOPLE IN HEALTH CARE

The Registered Nurses Association (RNAO) in partnership with Indigenous Cultural Safety, Indigenous Primary Health Care Council (IPHCC) and the Indigenous Nurses and Allies Interest Group (INAIG) invite you to join a webinar entitled "Turning Knowledge into Action: Creating Safer Space for Indigenous Peoples in Healthcare" on September 26, 2023 from 1 - 2 p.m. (ET).



The objectives of this webinar are to:

  • Define Indigenous cultural safety and explain its importance in health service delivery;
  • Recognize ways in which colonization has adversely impacted the health and wellbeing of Indigenous people;
  • Demonstrate an understanding of the legacy of Indian Residential Schools, and acknowledge the ways in which it created this cycle of intergenerational trauma for survivors, their families and their communities;
  • Commit to developing a critical consciousness and the knowledge and skills necessary to work in a culturally safe and trauma-informed way while uplifting and supporting Indigenous nurses; and
  • Identify potential next steps that can be taken in order to continue your learning journey and get involved.

LAWSON HEALTH RESEARCH INSTITUTE: CARE MANAGEMENT & COORDINATION OF OLDER ADULTS

Does your work involve the coordination and management of care for older adults?


The Lawson Health Research Institute team is gathering information about care management and coordination roles across the system, with an effort to improve health system integration for older adult patients, their caregivers and health care providers.


Participation involves a short (~30 Minute) interview with a researcher, conducted

remotely.


Compensation will be provided for participants whose time would otherwise be

uncompensated.

LONDON'S BUILDING SAFER COMMUNITIES FUND GRANT PROGRAM

London’s Building Safer Communities Fund Grant program supports the development of new, youth-focused prevention, intervention, and transformation projects aimed at reducing gun and gang violence in London. The objectives of London’s Building Safer Communities Fund Grant are to:


  • Effectively address risk factors that may result in a young person engaging in gun and gang violence.
  • Enhance protective factors that may result in a young person refraining from or exiting involvement in gun and gang violence.
  • Identify and support innovative, evidence-based practices and programs to prevent or address youth involvement in gun and gang violence.


In total, there is $2,783,933 in funding available through London’s Building Safer Communities Fund Grant program for December 2023 to March 2026. Applicants can request funding for one, two, or three years.


Not-for-profit registered organizations located in London and grassroots groups whose applications are hosted by a registered not-for-profit organization located in London are eligible to apply.


The application deadline is October 9, 2023, at 11:59 p.m.

BOUNCE BACK & THRIVE

The Bounce Back & Thrive (BBT) program is a resiliency skills training program designed to meet the needs of parents (and those in a parenting role) of children under the age of 8. BBT helps parents gain self-regulation and thinking skills that increase their capacity to provide caring relationships and role model resilience-building skills in daily interactions with their children. Then, it shows parents how to directly introduce basic resiliency skills to children.

THE HEARTLIFE FOUNDATION: 2023 ANNUAL E3 PATIENT DAY
