WHOLISTIC NEEDS SCREENER: CO-DESIGN SESSIONS | |
London Health Sciences Centre (LHSC), in partnership with the Middlesex London Ontario Health Team (MLOHT), would like to invite you to take part in a co-design session for a Wholistic Needs Screening tool.
The goal of the screener is to enable a more preventative approach to managing individuals' health and social needs and to connect people more easily to the most appropriate services for them. We are looking for community members and service providers to help develop and review the questions and the format of the screener.
There are two virtual sessions currently scheduled.
Evening Event- Tuesday, April 25th at 7:30-8:30pm
Lunch event- Thursday, April 27th at 12:30-1:30pm

HEALTH & HOMELESSNESS IN LONDON, ONTARIO: A WHOLE OF COMMUNITY SYSTEM RESPONSE | |
The "Health and Homelessness: Whole of Community Response" initiative is aimed at addressing the complex issue of homelessness in London. The response involves a collaborative effort between various stakeholders, including healthcare providers, social service agencies, and community groups, to provide comprehensive support and services to individuals experiencing homelessness.
The initiative focuses on four key areas: prevention, outreach, housing, and support services. The goal is to provide a continuum of care that addresses the immediate needs of individuals experiencing homelessness while also working towards long-term solutions.

CALL FOR CO-DESIGN PARTICIPANTS: METHAMPHETAMINE PATHWAY | |
The London Middlesex Mental Health and Addiction Strategic Direction Office has partnered with the Office of Population Health Management and Health System Partnerships at LHSC and the Middlesex London Ontario Health Team to work on the development of a pathway for people who use methamphetamine or are at-risk of using methamphetamine.
The team is looking for people who interact directly or indirectly with people who use substances to join the co-design network to inform and guide the development. These care pathways will allow people who use methamphetamine to receive better care and not fall through the cracks. The time commitment is flexible and based on your availability.

MIDDLESEX COUNTY: REUSABLE HYGIENE PRODUCT INITIATIVE APPLICATION

Access to menstrual (period) products, like pads and tampons, can be a challenge for many people.
Starting February 2023, Middlesex County is offering a reimbursement of up to $50 per eligible person. The benefit applies to Ontario Works and ODSP clients who experience menstruation on the purchase of any reusable period product (washable pads, panty-liners, or menstrual cups).

PALLIATIVE CARE RESOURCES | |
The Division of Palliative Care - 3 Days in Palliative Care |
The aim of the program is to help participants:
- Manage complex physical, emotional, ethical and social issues, including pediatric to geriatric populations.
- Develop enhanced collaborative practice skills and forge new inter-professional relationships.
- Apply a palliative approach in end of life care for cancer and chronic end-stage illnesses, including cardiac, renal, respiratory and neurological conditions.

Intellectual and Developmental Disability (IDD) Palliative Care: Enabling "Good Practice" |
This session is intended to introduce the attendees to the distinct and
complex needs of someone living with IDD. Recognizing the importance of
integrating the Developmental Service Specialists as part of the health care
team. Introduction to IDD specific palliative tools will also be covered.
