Inequities in incidence and prevalence of diabetes and pre-diabetes have highlighted a need to focus prevention efforts on priority populations. This training will address the disproportionate effects of diabetes and pre-diabetes on certain populations of people. Learners will discuss defining and identifying priority populations in their communities and explore strategies to engage with these populations.

American Medical Association (AMA) Webinar

96 million adults have prediabetes, a serious health condition characterized by blood glucose levels that are higher than normal, but not high enough to be diagnostic for type 2 diabetes. Among adults with prediabetes, only 19% report being told by a health care professional that they have this condition. Primary care providers (PCPs) along with their care teams and healthcare organizations play critical roles in preventing type 2 diabetes, particularly among patients with prediabetes. Processes that improve care include systematically identifying individuals with prediabetes, engaging individuals in shared decision-making regarding evidence-based treatment. Gina Aquino and Jill Yore from Henry Ford Health System will discuss how they integrated their screening and referral process for diabetes prevention into their Electronic Health Record (EHR).
