FHCSD'S PACE Program:

Here to Help with Aging Loved Ones

According to US census data, the senior population in San Diego County will reach over 1 million by 2030. Specialized programs cater to the unique needs of seniors across San Diego County to help families with aging loved ones, but many do not know about them.

Family Health Centers of San Diego (FHCSD) has provided health services to underserved and low-income San Diegans for over 50 years. One of FHCSD's most outstanding programs is the Program of All-Inclusive Care for the Elderly (PACE), designed to address all health care needs of our seniors. In 2022, our PACE program provided comprehensive support to hundreds of older adults.

FHCSD’s PACE is a comprehensive health care program providing seniors with coordinated medical care, transportation, fresh food and social services. “PACE is an outstanding alternative for families who are struggling to care for a parent or loved one who needs in-home services and would benefit from their medical care being managed by a team with a geriatrician, RNs, social workers, and other clinicians.” said Jeffrey Gering, FHCSD’s Senior VP. FHCSD PACE coordinates and provides access to primary care physicians, specialists, and other health care professionals who are specially trained to work with seniors. Transportation is provided to our vibrant Day Center and to approved medical appointments where many services, not available in a traditional health care plan, are offered.

At the Day Center, recreational therapists, specially trained to improve the well-being of participants, use a variety of activities and techniques such as art, crafts, dance and music. The impact of the Day Center on seniors can be dramatic. Javier, a PACE participant, felt he didn’t need help. His sisters urged him to enroll in PACE to help his health and lift his mood “PACE helps me become more independent by providing activities at day center. I do feel more independent. … “What I love about the PACE program is the music activities, I meet new people and I really like the food (which) is delicious!” Javier’s family sees positive changes in him, he has a good, uplifted spirit and they are happy for him. A participant with diabetes came to FHCSD PACE for help with controlling his blood sugar. PACE helps him with nutritional education on portion size, sugar intake and the on-site kitchen staff prepare meals that are customized to his dietary needs. “The food is good, and PACE helped with my diabetes. The dietitians have taught me how to control my portions and how to control my blood sugar with what I eat.” Our participants appreciate the support provided by their care teams.

FHCSD PACE has locations in mid-San Diego and Oceanside. Many PACE participants live alone, but need support, some live with family members who work full-time and can’t coordinate the many doctor appointments.

As the senior population in San Diego continues to grow, FHCSD PACE is here to offer specialized programs that cater to older adults’ unique needs. If you or a loved one is a senior in need of health care services, consider the PACE program at Family Health Centers of San Diego by visiting our website.

FHCSD PACE provides individualized care by an interdisciplinary care team of professionals focused on working with older adults so they can continue to live safely at home.
