Dear SBMS Parents and Guardians,

We are looking forward to welcoming our new students to SBMS! Our school uses the Magnus Health online management system for student medical information. As this is a web-based system, you will have continuous access to your child’s health records and the ability to make updates when needed. There are many benefits to using a secure online system, including less paperwork for parents to complete and more accurate information. 

To access your Magnus Health Account:

• Login to the SBMS Hub
• Click on the “Magnus Health Sign On” tile on the Resources board
• New parents - once logged in, please watch the short video intro
• Let Anne know if you need help logging into the Hub

We ask that you enter the health information required within your Magnus Health account for each child attending SBMS no later than Monday, August 12, 2024. Please reserve 20 to 30 minutes per child to enter or update the required information and upload the documents. Be sure to schedule the medical evaluation for your new student soon because there can be a long wait for appointments in the summer.

Requirements for ALL New Students: Due by Monday, August 12, 2024

• Medical Evaluation Form - only for new students (filled out by their pediatrician)
• Vital Health Record (includes Health Emergency Contact Info)
• Over-the-Counter Medication Form
• Treatment Authorization Form
• Medical Insurance Info/picture of the card
• Immunization Records and Immunizations Current
• Prescription Medication Form- if applicable
• Asthma Action Plan Form- if applicable
• Allergy Action Plan Form- if applicable
• Diabetes Action Plan Form- if applicable
• Seizure Action Plan Form- if applicable
