Disclosure of Ownership and Cultural Competency Requirement

In accordance with regulatory requirements of the Minnesota Department of Human Services (DHS) and the Centers for Medicare & Medicaid Services (CMS) regulations, all payors, including Fulcrum Health, Inc. (Fulcrum) must collect and maintain information regarding disclosure of ownership and cultural competency from all providers in its network on an annual basis.

 

Deadline extended! - Fulcrum is requesting that each clinic complete the online form by April 15, 2023.

MN RETAIN - Now Expanded to Select Gold Tier Providers!

Centers of Excellence Clinics and select Gold tier network providers in Fulcrum's ChiroCare network are invited to participate in the federally funded statewide study for injured or ill patients that are on restriction or not able to work.


MN RETAIN (Retaining Employment and Talent After Injury/Illness Network) is working with injured or ill Minnesota workers who are on restrictions and not able to do their usual work, or providing support so they can return to work as soon as possible. 


The goal for Fulcrum providers is to refer 312 accepted patients by May 2024. The hope is that each provider will refer one patient every other month. Patients who are accepted and assigned to the intervention group will receive a $100 gift card per patient.


Dr. Deb Zurcher will be supporting Fulcrum chiropractic providers on this initiative until May 16, 2024. Reach out to Dr. Zurcher at d.zurcher@fulcrumhealthinc.org for any questions or materials.


Fulcrum was awarded subrecipient participation in the MN RETAIN Phase 2 study, a federal grant study funded by the U.S. Department of Labor and the Social Security Administration. The Minnesota study is a collaboration among and between the following organizations: Governor’s Workforce Development Board, Mayo Clinic, MN Department of Employment and Economic Development, MN Department of Health, MN Department of Labor and Industry, and Workforce Development, Inc.

Accepting ChiroCare Centers of Excellence Applications!

We are now accepting applications from ChiroCare network providers who believe their clinics meet the Centers of Excellence criteria. The application period is extended until April 30, 2023.

 

The CCoE Self-Assessment, Application, and Attestation can be completed online for administrative ease.

 

Please log into fulcrumproviderportal.com, go to Learning Hub tab and then the CCoE tab to access the necessary documents to apply and take the self-assessment.

 

If you have completed the CCoE self-assessment and received a passing score of 20 or higher, please consider completing the CCoE application.

 

Current CCoE's do not need to re-apply. Verification review of records began February 2023.

Reminder: Access Fulcrum's Provider Portal Frequently for Important Resources and Information

View your customized provider portal today by entering your unique email address!



As a provider, what do I need in order to access my customized portal?

  • Each provider is required to provide Fulcrum with a unique provider email address which will become your user name to log into the portal. This email address cannot be shared with other providers.
  • If you would like to add a billing company, admin, or anyone else to view your portal account, please use the Add A Team Member feature available within the portal.


Why do I need a unique email address to access my provider portal?

  • For privacy reasons, each provider must have his/her own user name and password for logging in. The portal uses the unique email address you provided to Fulcrum for the provider portal user name.
  • Since the portal is customized to each provider, it is critical for privacy reasons that the provider decides who is allowed to access and view his/her account.
  • Additional team members identified by the provider can view the customized portal by allowing permission in the Add a Team Member feature inside the portal. This feature also allows the provider to remove any or all permissions at any time.


If I need to change my email address that I have already provided to Fulcrum, what do I do?

  • Please email Credentialing@fulcrumhealthinc.org for a change to your email address and then once you receive a confirmation of the change, please create your portal account.


When should I create my portal account?

  • Please create your account as soon as possible to view the most up to date resources including plan summary reference sheets, fee schedules, and much more!


How do I create a Fulcrum Provider Portal account?


Its easy! Just follow these steps:

  • Step 4: Familiarize yourself with the portal, customized for you!

Changes to Clinic and/or Provider Information? Let us Know!

The federal No Surprises Act requires health plans to verify the accuracy of provider contact information every 90 days. Fulcrum will provide more information about this requirement and what it means for you and for Fulcrum in a future communication.


Currently, your provider agreement with Fulcrum requires you to immediately notify Fulcrum of any change in your or your clinic’s demographic information, including, but not limited to name, address, phone, fax, email address, and taxpayer identification number.


Fulcrum’s Provider Portal offers a quick and electronic method for reporting changes.  Simply select the Demographic Updates tab on the home page.
