Two of Lifewell Behavioral Health Wellness clinics are moving to a new location on or after July 1, 2020;
- Lifewell Royal Palms clinic: 2311 W. Royal Palm Rd, Phoenix, AZ 85021 (2.8 miles)
- Lifewell Beryl clinic: 2505 W. Beryl Ave., Phoenix, AZ 85021 (0.9 miles)
The new location these clinics are moving to is:
- Lifewell Desert Cove clinic: 10844 N. 23rd Ave., Phoenix, AZ 85022
This new clinic will serve members with both Serious Mental Illness (SMI) determination and General Mental Health/Substance Use (GMH/SU). There will be no change in service at the new clinic.
The new location is less than 3 miles from the current clinics and is on a bus line. Mercy Care and Lifewell Behavioral Health Wellness are working diligently to ensure all members, and their families are notified of these location changes. Member and family outreach will be made. In addition to regular face-to-face contact, members and families will receive a letter informing them of the new location. The letter informs members and families that unless a selection is made to go to a new clinic, they will be automatically transitioned to the new Lifewell Desert Cove clinic location. It's important that we all work together closely to successfully transition members and ensure that they receive the high-quality, comprehensive care that they need and deserve.
