You have been invited to participate in a research project on The Exploration of Patient Education In Responsible Opioid Use by Dental Surgeons, which is being conducted by a student investigator as part of her AP Capstone Research course at Assumption High School in Louisville, KY.

This is a 9-question voluntary survey that will take less than 5 minutes of your time to complete.

The consent document at the beginning of the survey will explain the purpose of the research project and will go over all of the time commitments, the procedures used in the study, and the risks and benefits of participating.
