On the two year anniversary of the Dobbs decision (overturning Roe v. Wade and leaving it up to the states to decide if abortion is legal), HealthAdvocateX is hosting this month’s Community Conversation about the impact of the Dobbs decision on women’s health care in the United States. As a non-political organization, you might wonder why we selected this topic? I have been reflecting back about the founding of HealthAdvocateX in 2012 (as the Washington State Health Advocacy Association – WASHAA) and what we set out to do at the beginning. We sought to help forge a new field where patients and families could get assistance to navigate the maze of our health care system, according to what was best for themselves, not what the healthcare system picked for them. Our organization has consistently supported individuals being able to access the care they need and deserve and to educate and make more visible areas that are often misunderstood, not highly resources or are not equitably available. We believe that ALL people deserve the right to access and utilize healthcare.
That is why after spending two years exploring our values in 2022, we published our IDEA statement and in 2023 we launched the XCenter Series on Transgender Care.
We hope you will join us for our Community Conversation to learn more about how the Dobbs decision is affecting women’s healthcare and why this particular decision is costly ALL of us dearly in terms of unequal impact on healthcare resources and presenting increased barriers to equitable access to care.

The Dobbs Decision has significantly impacted women’s health care, particularly in states with abortion restrictions. Medical school and Ob/gyn residency applications have declined in these states, contributing to a shortage of practitioners. This exodus of Ob/gyns from already under-served areas like Idaho, Texas, and Louisiana exacerbates healthcare disparities. Additionally, the decision threatens the availability of infertility and cancer care in these regions, further complicating women’s access to comprehensive medical services. | | Lisa M. Barroilhet, M.D. – Dr. Barroilhet is an Associate Professor with tenure in the department of Obstetrics and Gynecology at the University of Wisconsin School of Medicine and Public Health. She completed her four-year residency in obstetrics and gynecology at University of North Carolina-Chapel Hill. After finishing her residency in 2009, she completed her three-year clinical and research fellowship in gynecologic oncology at Brigham and Women’s Hospital in Boston, Massachusetts. Dr. Barroilhet joined the University of Wisconsin Department of Obstetrics and Gynecology in 2012, where she currently serves as the director of translational research. She previously served as the interim chair of Obstetrics and Gynecology in 2022. She has been the recipient of the NCI Clinical Care Team Leadership Award, a K12 career development award, and is currently a GOG Scholar Investigator. Her laboratory is currently funded by the NIH and focuses on the use of small molecule inhibitors to prevent ovarian cancer.

This case review highlights crucial teaching points: not all cancers are terminal, emphasizing the importance of individualized prognosis and hope. It underscores the necessity of aligning treatment plans with the patient’s wishes, ensuring their autonomy and comfort. Additionally, it advocates for routine psychiatric evaluations for patients enduring long hospital stays, addressing mental health to enhance overall patient care and outcomes.
Learning Objectives:
1) One cannot assume all cancer diagnosis by name as there are certain subtypes with different outcomes.
2) Clients’ wishes of treatment always come first, even before family/doctor/advocate.
3) Always ask if a psych evaluation would be beneficial for a client with a long hospital stay.
4) If a client continues to have a recurrent infection, ask for an explanation.

Dr. Chris Price practiced as a general cardiologist for over 20 years until he retired in 2017. While working at The Everett Clinic and EvergreenHealth, he served in leadership positions including Department Chair and Director of Non-Invasive Cardiology. He has always been a dedicated patient advocate, whether that was for his own patients or his family friends. After experiencing a personal health issue that led to his early retirement, he became even more interested in patient advocacy and started his own business, Call Doctor Chris, to continue this passion and serve others in Washington state and beyond. He received his MD from Creighton University School of Medicine and his BS from Santa Clara University.

Last year we launched a five-part educational series on Transgender Advocacy that is now available for viewing and is also approved for Justice Equity Diversity and Inclusion (J.E.D.I.)/Ethics continuing education for Board Certified Patient Advocates. The concept behind these centers is to provide credible, understandable and actionable information on a variety of topics through the lens of health advocacy. Our first topic: Transgender Advocacy 101.
