Help Spread the Word: Ten Things You (and Your Loved Ones) Should Know about Home-Based Care

HCCI will host a free webinar at 12pm CT Thursday, June 8, to increase public awareness of home-based primary care entitled The Return of the House Call: Ten Things You (and Your Loved Ones) Should Know About Home-Based Care. The hosts will talk about the benefits of house calls and what they mean for patients and caregivers in our post-pandemic world. Please share this information with your communities and networks, including liking, commenting, and sharing HCCI’s social posts about the event, because more people need to know that house calls are back — and here to stay.

Free Learning Event for Illinois Hospice, Palliative Care Programs

Hospice and palliative care programs from Illinois and the surrounding areas are invited to attend a free HCCI learning event entitled, Bridging the Gap with Home-Based Primary Care. The event on Tuesday, June 13, from 4 to 6pm Central, will be held at the Hyatt Regency Schaumburg.


Participants will learn the benefits of home-based primary and home-based hospice and palliative care and bringing them together, and how to successfully incorporate both into a complex illness management model.


Afterward, participants are invited to stay for a reception honoring Champions from HCCI’s Illinois House Call Project.
