Dear MAP Families,

We hope you are having a GREAT SUMMER! The first day of Jump Start MAP is Tuesday, September 3, 2024. Below, please find lots of important MAP information and forms to complete for 2024-2025.

MAP is asking all families to complete some important paperwork, and have it back to MAP by WEDNESDAY, JULY 31, 2024. This includes:

• 2024-2025 CHILD INFORMATION FORMS – New child information forms must be completed each year. They include important information about your child, family contact information, and authorizations.(different from the registration form). They are in a fillable PDF that can be downloaded, completed, saved and emailed directly to your child’s program director. It can also be downloaded directly from our webpage: www.medfieldafterschoolprogram.com. If using a mobile device, it is recommended that you download the Acrobat App to complete the form. You can also print, complete and mail the completed form to MAP P.O. Box 18 Medfield MA 02052.   CLICK HERE for the 2024-2025 CHILD INFORMATION FORM MAP must have your child's completed form by WEDNESDAY, JULY 31, 2024.

• DEVELOPMENTAL HISTORY & CURRENT PHYSICAL: Jump Start Families must complete a Developmental History Form and provide MAP with a current physical. Both may be emailed along with your Child Information Form and are due back to MAP by 7/31/2024. CLICK HERE for the DEVELOPMENTAL HISTORY FORM.

HEALTH PLANS: MAP's Health Care Policy & required forms are available on our web site: www.medfieldafterschoolprogram.com. Families of children with any chronic medical or health condition which has been diagnosed by a doctor or licensed health care practitioner, including but not limited to serious allergies, anaphylaxis, asthma, ADD/ADHD, celiac disease, diabetes, epilepsy, physical disabilities, etc. which may or may not require medical treatment and/or medication at home, school or MAP are required to complete a MAP health form and to contact their child's program director (below). Once forms are completed, families must set up a time to meet to review the forms, discuss your child’s condition, provide training, and drop off medications (if any) between August 12-21, 2024. For the safety of the children, they may not attend MAP without their completed forms & medication (if any). Need to make a Change? If you are electing to make a change to your day(s) of attendance or withdraw your child from MAP prior to the beginning of the 2024-2025 school year, please notify MAP in writing via email to annette.map@comcast.net. MAP will adhere to the following tuition deposit guidelines:

• 50% refundable if we are notified in writing via email by August 15, 2024
• Non-refundable after August 15 and families are responsible for their first tuition payment in full.
• Schedule changes will not be permitted August 15 through September 15.
• After September 15, 2024, MAP requires a two-week written notice to make schedule changes (add days based on space availability, drop days, or withdraw from a special sign up or the program). 

Tuition Statements: Families will receive their first Tuition Statement from MAP via email from quickbooks@notification.intuit.com on August 15, 2024. (please save to your address book and check your spam or promotions folder) Payment is due to MAP on or before September 1, 2024. Per our tuition policy (available at www.medfieldafterschoolprogram.com): Tuition is due in advance. Tuition payments are payable to the "Medfield Afterschool Program" or “MAP.” Payments must be by check, via your bank’s online bill pay, a money order, or ACH (see below). Please put your child's name on your check. Tuition payments can be mailed to: MAP, P.O. Box 18, Medfield, MA 02052, placed in MAP’s secure drop box at the 2-3 MAP building/Main Office (located behind the Wheelock School, door closest to the playground), or dropped off at your child’s program (once in session). 

ACH Payments - MAP offers families the opportunity to have their monthly tuition and any related MAP expenses debited directly from their bank account via an electronic bank to bank payment called ACH (Automated Clearing House). In order to participate in this optional form of payment, families must complete and return the Authorization Agreement and provide MAP with a cancelled check. MAP families will continue to receive a monthly tuition statement on the 15^th of each month, and tuition payments will be due on the 1^st of the following month. ACH payments will be drawn from participating families accounts in batches, starting on the 1^st and may occur within the first 5 business days of the month.

To enroll, please click below. Families may enroll at any time. Enrollment forms must be received by the 15th of the month in order for payments to be processed on the 1st. Enrollment forms and your voided check should be returned to MAP in a sealed envelope. MAP will confirm your enrollment via email.
