Dear SETSS Provider,
NYCDOE recently sent out an email to the special education teachers who are listed on the SETSS Independent Provider Registry (also known as the Municipality List Or Directory) asking them to complete a short survey that verifies their information. The email was sent from relatedservices@schools.nyc.gov. Please see below if you did not receive this email.
Please be aware that inclusion in this registry means that you agree to accept the NYCDOE’s standard rate of pay. See our memo here regarding the new rates.
Failure to respond to the email from the DOE may cause you to be deactivated in PETS, which is a list of all DOE service providers who have passed background checks and fingerprinting. Deactivation will result in being unable to work for the NYCDOE, both directly and indirectly, i.e. through an agency.
Instructions for completing the survey in the DOE email:
If you would like to be included in the registry, and will accept standard DOE rates:
- Answer yes to Question 3.
- Answer yes to: Confirm Continued Interest in Directory Listing
If you would like to be removed from the registry and remain active in PETS:
- Answer yes to Question 3.
- Answer no to: Confirm Continued Interest in Directory Listing
- Answer yes to: Confirm Removal From Directory
If you would like to be deactivated in PETS and no longer be able to work for the NYCDOE:
- Answer no to Question 3.
- Confirm that response on the next page.
If you did not receive the email, and would like to be included in the registry, we suggest that you complete the following application by clicking here. If you would like to be included in the registry, and will accept standard DOE rates, then check yes on the Municipality List question.
