Dear Parents/Guardians and students,

Welcome to the kick-off for the 2024 – 2025 school year. This is hard to believe since we have just recently finalized grades, report cards and honor roll for the 2023 – 2024 school year! Nevertheless, we are working and planning already! Later this summer, we will open up the Summer Documents website that will provide all of the information about next school year.  Today, we are beginning our annual data update. Read more below.

As you know, MND uses a secure, online site (FinalForms) to update our database and to complete required items for attendance at MND for each school year. FinalForms gives families the ability to submit and update their demographic data, as well as review, fill out and sign important school documents. FinalForms stores this information, and it can be updated and changed throughout the year. You can log back in at any point to update information. MND will be notified when those changes occur. Every summer, we must review and update the data, so that we have the most accurate information for your daughter. Please review all pages, so we have a more complete database.

As a reminder, when all forms are complete and signed, you will see a "Forms Finished" message. An email will automatically be sent to the email address on record for your daughter, prompting her to sign any forms requiring a student signature. We have eliminated student signatures from most forms. However, students will need to sign off on the Tablet PC Acceptable Use Policy form and on some athletic forms if you are an athlete. 

NOTE for New MND Families: Your daughter will NOT be able to sign off on her portion of the forms until she receives her Tablet PC later this summer. For now, simply complete the parent/guardian portions.

PARENTS OF CURRENT OR POTENTIAL ATHLETES FOR 2024 - 2025:

If your daughter is playing/considering playing ANY MND SPORT this year, you must add the sport(s) by clicking on "Add Activity" from the main FinalForms page. There are additional forms that will appear for athletes. Please add the sport if it is possible you will play. You can always delete the sport at a later time. Most athletic forms are digital. However, the OHSAA Physical Form must be printed, completed by the family and doctor and returned to MND's Athletic Office or uploaded to the portal. Once the record is received at MND, the physical expiration date will be entered, and the system will generate automatic reminders to you for future years. Families of athletes are asked to finish this process by July 31, 2024.

TABLET PC INFORMATION FOR RETURNING STUDENTS:

This year, MND has transitioned our summer reimaging process to a new method. Tablet PCs for Classes of 2025, 2026 and 2027 maybe picked up at MND beginning Monday, July 8 as follows:

• Arrive to the Main Office between the hours of 8:30 AM and 11:30 AM
• Remember to bring your Tablet PC case!
• Your FinalForms for the parent and student have been completed.
• Please remember that ALL FINALFORMS FOR PARENTS AND STUDENTS must be completed in order for a student to pick-up her Tablet PC. 

NOTE: Students/families whose Tablet PC has been sent out for service will be notified via email when their Tablet PC is ready for pick-up.
