Sixteenth Street Community Health Centers, a valued foundational partner that has supported the creation and expansion of IMPACT Connect, is now officially on the referral network! Through full-service bilingual clinics, a behavioral health clinic, a satellite geriatric clinic, and in-school clinics, Sixteenth Street cares for nearly 43,000 primarily low-income, Latino or diverse individuals each year.
Welcome new partners
IMPACT Connect welcomes Shorewood Senior Resource Center which promotes safe, healthy and enriching lifestyles for residents age 60 and older. Membership in IMPACT Connect will enhance their efforts to serve as a connection point for senior programs and services.
Gervonni Cares also went live this month. The community action organization supports a range of needs including financial literacy, eviction prevention, utility support, food insecurities, homelessness, and vehicle repair.
