We are proud to announce that both Heritage Valley Beaver and Heritage Valley Sewickley have earned a 2023 HeartFlow CT Quality Award for Excellence in Coronary CT Imaging. Recipients of the award are in the top 20% of global medical centers providing patients with a better pathway for identifying and understanding their heart health by utilizing coronary CT and the HeartFlow Analysis.

The HeartFlow Difference: The HeartFlow Analysis is a non-invasive way of diagnosing Coronary Artery Disease (CAD) with even greater accuracy. This cardiac test provides a personalized, color-coded 3D model of your coronary arteries showing how the blockages are impacting blood flow. The HeartFlow Analysis is the first and only non-invasive test that enables your physician to understand the impact that narrowings and blockages have on blood flow to your heart – information that otherwise would only be available with a riskier, invasive procedure. Your doctor will order a standard CT scan of your heart to look for CAD. If the scan shows narrowings in your coronary arteries, your doctor may order a HeartFlow Analysis.  No additional visit is needed for this test.

Linda and Megan are two important members of Heritage Valley’s Population Health Team. Linda is the Social Worker and has over 25 years of experience. She is community-based, and often meets with patients and families in their homes to assess their needs and coordinate services. She has a passion for making a difference in people’s lives.

Megan is the Pharmacist on the team, and has been with Heritage Valley for 22 years. She especially enjoys communicating directly with patients to address medication concerns. Performing complete medication reviews and making recommendations for optimizing medication regimens is a big part of helping patients feel their best and achieve better health.

Both Linda and Megan, along with the rest of the Population Health Team, work together to address the needs of patients, some with complex medical and social needs, and develop the best plan of care for each one. It is not uncommon for patients to require the entire team’s expertise to meet their needs. For example, a patient could require Diabetic Education to better understand a Diabetes diagnosis, but at the same time be struggling with an economic hardship. The team takes a holistic approach to caring for patients, and does whatever is necessary to ensure stability and improve quality of life.

Linda and Megan both agree that no two days are the same, and that they find great satisfaction in helping our patients. They also agree that their team is “amazing.”  “I am honored to be part of the Population Health Team and am so proud of the work that we do on a daily basis for our patients,” states Linda. Megan adds, “I feel privileged to work on the Population Health Team and collaborate with such excellent team members to provide individualized care for our patients.”
