Changes in your Coverage?
Any changes due to a birth / adoption, marriage, separation or divorce, must be updated under insurance coverage.
An Employee Change form, or submission of an Employee Change on-line through www.my-benefits.ca, must be received at the Chambers Plan within 60 days of the change.
If the 60 day deadline is missed, the employee may be required to provide medical evidence of insurability and be approved by the insurance company. Changes take effect on the date of the event.
