Dear Surgical Residents,


Greetings for the month of July! Hope you have all hit the ground running on your respective first rotations of the academic year. No matter how long you have been in residency, there is always some adjustment to your new role(s) at the beginning of the year. In a way, residency is often about learning how to do a new job every year: your responsibilities change; expectations are different; your perspective on work and life evolve. There is always some amount of discomfort, but I would encourage you to embrace working just at (or slightly beyond) the edge of your comfort level. This applies not just during training, but during the rest of your career as well.  

 

A few notes from the program:


Departmental M+M: 

Next Departmental M+M is Wed July 10, 2024

As many of you noted last week, we have started a revised departmental M+M format. We wanted to make the cases more timely, more focused on educational points with less PowerPoint and more discussion (particularly among faculty). For M+M only (not Grand Rounds), all attendees must either come in person to Parnassus or the designated room at the respective clinical sites—SFVAMC, SFGH, and Mission Bay (we are working on CPMC and Kaiser). Food will be available at each site. When you are asked to present, we want the presentations short and to the point. We hope this will be the most educational format for everyone involved.

 

Town Hall 7/17/24

The following week we will be having our first general surgery residency town hall. This will be held in-person only at Parnassus. We will start with 15 minutes of discussion/presentation by program faculty leadership, and for the rest of the time, the administrative chiefs will hold an open discussion with residents only. Please make every effort to attend. 

 

Feedback to the program

As we move forward during the academic year, there are sure to be issues (large and small) that come up on your rotations. We want to make sure that all residents are aware of ways to communicate concerns or issues with the program. These issues can be brought up anonymously if preferred, although this makes it difficult to "close the loop" with the person that brought up the concern. Here are some of the ways in which issues can be brought up:

 

 

Case log guide

Just a reminder to everyone to keep your case logs up to date! This is very important for your progression through the program and ultimately, board certification. Please click here for a slide deck about some pointers about case logs. If you have 20 minutes, feel free to watch this video that I recorded earlier about case logging.  

 

Just a few more notes about case logging: 

  • Surgeon Chief (SC): credit during 12 months of chief year ONLY
  • Surgeon Junior (SJ): credit during all other years of training
  • Teaching Assistant (TA): when a senior resident is working with another resident who will count the case as “SJ”.
  • TA cases count toward TOTAL MAJOR, but not SURGEON CHIEF totals. Minimum 25 
  • First Assistant (FA): when resident is acting as an assistant and not the primary surgeon
  • Most cases where one resident is scrubbed with the attending should be SJ
  • FA cases do NOT count toward TOTAL MAJOR

We are thrilled to introduce Briseis Schreibman, who has joined the Education Office as the new HR Analyst as of July 1st! Briseis will have primary responsibility over appointments and other HR functions in support of all residents in the Department of Surgery.


Briseis is an avid reader, an enthusiastic museum-goer, and a weekend adventurer. She is looking forward to meeting and collaborating with all the residents. Please feel free to reach out to briseis.schreibman@ucsf.edu if you have any questions or just want to say hello.

Resident Assessment & Evaluation


Questions about performance assessments and evaluations? Check out the presentation given during intern orientation by Dr. Lan Vu HERE, which summarizes and defines ACGME Milestones and Entrustable Professional Activities for general surgery, as well as how feedback works both to and from residents in the program. If you are interested in learning more, check out the ABS Webinar for general surgery interns.


Regarding EPA assessments: Congratulations to the top 6 residents (Mark Barry, Axel Gomez Casarez, Basil Karam, Sophia Hernandez, Keith Hansen, and Mohammad Karimzada) who had the highest number of completed EPA assessments for the 23-24 academic year! The goal for the 24-25 academic year is for each resident to have at least 8 EPA assessments completed per rotation (except Neurosurgery, CT surgery, skills lab/jeopardy rotations.) This metric will be tracked and shared monthly in Hirose-Grams and at Grand Rounds. A gift card will be given to the resident with the highest number of completed EPAs each month. Look out for an email in the next couple of months with a link to your personal EPA dashboard to help track your progress. More details on EPA assessments can be found HERE on the Resident Portal.

Upcoming Town Hall


Please save the date for the first resident town hall of the new academic year on Wednesday, July 17th at 7am in room S-214 (513 Parnassus Ave).The goal for these town halls is to provide a forum for communication between residents and program leadership; we hope this provides another avenue for you to voice questions or concerns. If you would like to anonymously submit questions or discussion topics in advance, please fill out this form.

A Little More About ACGME Case Logging


Start the year strong by getting into a regular habit of logging your cases! Look out for a notification in the next few days letting you know that you can start logging your 24-25 cases - until then, keep separate track of them until the ACGME case log system is available to you.

Transportation While Fatigued


An important element of resident wellbeing is safe travel when fatigued. There are several funded transportation alternatives for residents who find themselves too tired to safely drive home after work:


GME-Funded Programs

  • GME provides a taxi voucher program at ParnassusMission Bay, and the VA - details can be found HERE
  • Residents should receive an email from Yellow Cab of SF prior to ZSFG rotations to download the YoTaxi app, which can be used to book rides from ZSFG when fatigued. More information can be found HERE (and please contact Kate Goldman with questions.)

Department-Funded Program

  • For all other sites (or if the GME funded programs are not easily accessible), please take an Uber or Lyft home and submit your receipt for reimbursement for both the ride home and the ride back to the hospital site. While there is currently no limit on how often a resident can use this program, resident use is monitored to ensure that we don't exceed departmental resources for this service. 
  • To be reimbursed for department funded fatigue travel, complete the Reimbursement Form by selecting Reimbursement Type Fatigue Travel. This form can be filled out from your phone, but you must save the receipt from your email to your phone files and upload. When you submit your receipts, please ensure that it has the start and end times and locations. Please bookmark the link below for quick access (it can be found under the Policies tab of the Resident Portal)

All residents are encouraged to attend monthly Surgical Education Committee (SEC) and Professional Development Committee (PDC) meetings. (Outlook invites to new residents coming soon!) Resident representation and participation allows you to voice your ideas and concerns, as well as learn about ongoing program improvement efforts. Thursday Lunches in the Education Office


If you are at Parnassus campus on Thursdays, don't forget to stop by the Chair's Office Conference Room (Room S-322) between 12 and 2pm to grab some food, as well as chat with Dr. Hirose or write a lunch note if you have time. Anyone who is available and hungry is welcome to attend!

Second Thursday


We are kicking off our monthly "Second Thursdays" for the 24-25 academic year! We look forward to seeing everyone at the back patio of Underdogs Tres at 9th and Irving on July 11th, 7:30-9:30pm (note time change this month). All residents (especially new ones!) are welcome. Respond to the Outlook meeting invitation to confirm your attendance.
