Find House Call Providers in Your Area

Our staff is working tirelessly to increase access to home-based primary care for the almost 6 million homebound and medically complex individuals in the U.S. who cannot currently access the care they need from the comfort of their homes. As part of these efforts, the Home Centered Care Institute, National Home-Based Primary Care Learning Network, and Duke-Margolis Center for Health Policy partnered with the American Academy of Home Care Medicine, in their development of The House Call Finder. The House Call Finder is a free, national practice directory dedicated to helping communities access home-based medical care.

 

This tool is important for everyone — whether it’s you, a loved one, a friend or neighbor, or anyone else who needs care in the home. Together, we can work to share the importance and impact of home-based primary care so that everyone can get the care they need in the comfort of their own home or place they call “home.”

“It’s a Dream to Me”


We spoke with Dr. Thomas Cornwell, Executive Chairman, HCCI, to discuss why house calls are so important to him and why he’s made it his life’s work to expand access to home-based primary care. During the conversation, he said he is still amazed at the impact that house calls have on homebound patients and their caregivers. He also shared the profound statistic that house calls can reduce hospitalizations and nursing home placements by 90%, and most importantly, in his words, “fulfill families’ wishes to keep their loved ones at home."

For example, HCCI’s HCCIntelligence™ Webinar: Unlocking Revenue Streams: Navigating the 2024 Medicare Physician Fee Schedule for Home-Based Medical Care today, Wednesday, January 10, 3 to 4pm CT, will review the coding, reimbursement, and policy changes specific to home-based medical care and how to best respond to these changes. To date, already over 140 people have registered, including providers, practice staff, and more.

HCCI Launches Illinois House Call Project Cohort 2

Approximately 260,000 individuals in Illinois are homebound, with only 26% of these receiving home-based primary care. The unmet needs in Illinois are significant. HCCI is excited to report that the first cohort of HCCI’s Illinois House Call Project has enrolled over 2,100 patients.

 

We are working to continue this tremendous impact by launching the second cohort for the Illinois House Call Project with the overall goal for the entire project to enroll 6,000 new house call patients by 2026. Be considered for early access to our Illinois House Call Project Cohort 2 or nominate a deserving practice here by Friday, January 12.
