We are so excited to announce that we now have a free Student Health Clinic on campus! Here are a few things to know about our clinic:
- The clinic is for SUU students only, it is walk-in only, and no appointments are needed at this time.
- The clinic is not an ER but offers services normally done in a primary care setting, similar to an instacare clinic.
- The clinic does not offer any schedule II medications.
- For any medications that the clinic prescribes, the students will have to pay for at the pharmacy of their choice.
- Services provided in the clinic are completely free, and no insurance is needed. Any additional services like referrals, labs, imaging, and things not done in the clinic, will have a cost.
HOURS: M-F 9:00 am to 5 pm
351 West University Blvd Bennion Bldg, Suite 111
Phone: 435.867.9412

If you need tax forms for the tuition you paid for your student, you can go here. Your student may need to set you up as an "authorized user". They can do that through their portal under "Student Billing Systems".
Students can view and print their 1098-T form in their MySUU portal. Under "Finance Menu", click the "Student" heading, followed by "Student Records", then "Tax Notification", then enter the tax year.
To check or update the permanent address on file, students can click on their name on the main MySUU screen, and then click on "Contact Info".
If your student has any questions about their 1098-T form, contact the Cashier's Office, ST 165, 435-586-7720.
If your student needs help filing their taxes, they can check out the
Volunteer Income Tax Assistance program.

1. Stop using all technology 30 min before bed.
2. No caffeine after 3 PM.
