Annual Wellbeing Assessment & Flu Vaccines with Northwestern Medicine
Wellbeing Assessment - (10 Reward Points)
District 300 has partnered with Northwestern Medicine to offer a Wellbeing Assessment that is FREE to all benefit-eligible employees. The Wellbeing Assessment includes a wellness screening and lifestyle survey. Information is confidential and visible ONLY TO YOU on Northwestern Medicine's Portal.
All benefit-eligible employees are encouraged to participate regardless of whether you enroll or waive the district’s health insurance coverage. Employees on the district health plan who participate in the wellness screening through Northwestern Medicine or provide evidence of receiving a wellbeing assessment from his/her physician will not incur a $240 Wellness surcharge in 2023. Registration is completed in Northwestern Medicine CHC Wellbeing's portal and is as easy as 1-2-3
Step 1 - Setup a Northwestern Medicine CHC Wellbeing Portal Account
An account must be created to track participation in the wellbeing assessment and rewards program.

Returning Participants - log in using your username and password

New Participants - click on the “New Participants” box, enter code: 5591Com143
Step 2 - Assessment Information - 9/1/2022 - 12/31/2022 (100 Reward Points)
In the portal, select one of the three options below to complete your wellness screening.

Onsite - screen conveniently at your school, see attached schedule (if a screening date is not visible in the portal it means that date is full, see FAQs #6 click here)

Through your Physician - choose one of the two forms below. Forms are located in Northwestern Medicine CHC Wellbeing Portal Portal.
- "Wellbeing Screening Results Physician Form” Form - includes blood panel results (form will generate after you complete the survey)
- *2022 Alternative Verification Form - does not include blood panel information (the form is located in NM Portal, once you sign in click on Settings then Benefits Center)
Step 3 - Complete the Health and Lifestyle Survey (50 Reward Points)
The survey will appear once you register for your assessment (Options 1-3 in Step 2). Please note: The remote form (Step 2, Option 2) or the Wellbeing Screening Results Physician Form (Step 2, Option 3) will not populate until you complete the survey. If you participate onsite, please complete the survey within 3 days of your screening date to view your results.
Onsite Flu Shot - (15 Reward Points)
In addition to the free wellbeing assessment, we will also be offering onsite FREE flu shots to all employees. No registration is required and will be available while supplies last. Walking challenges are back!

October Walking Challenge - (5 Reward Points)
Spooktacular Walking Challenge - October 12th - October 26th
Walk a total of 598 miles from Algonquin, IL to Pumpkin, GA from October 12th - October 26th. Registration ends October 11, 2022. For details and information on how to participate, click here.

All-Level Yoga with Toni - Thursday @ 4:15 pm (Limited space available)

Dates: 10/20 - 12/22 (no class 11/24)

Registration Cost: $45 for all 9 classes (capacity 24 participants)
- Suitable for a beginner who has consistent practice and for the seasoned yogi. This class will offer options for those that would like to explore their edge while simultaneously honoring students who may need more rest in a child's pose. All levels will offer modifications and challenges and the opportunity to focus internally on breath and meditation. This class will use both beginner and intermediate poses. Stress relief yoga creates balance in the mind and body and allows the participant to move within their skill set to avoid excess strain on the stretched muscles or joints. Slow, relaxing, and mindful, this class is a great way to slow down, reduce stress, connect with your body, and calm your mind. This all-level practice is ideal for anyone new to Yoga, but everyone will enjoy this opportunity to move their body, breathe deeply, and feel good. Bring a mat and water.

This month we would like to highlight Open Enrollment
Open Enrollment takes place every year from October 15th to November 15th. During this time you may make changes to your insurance coverage. District 300 participates in active enrollment, which means you must complete open enrollment to enroll, change, or waive insurance coverage that would begin January 1, 2023.
