CELEBRATIONS AND OBSERVANCES | |
Juneteenth is celebrated on June 19 to commemorate the end of slavery in the United States.
According to History.com, Juneteenth (short for “June Nineteenth”) marks the day when federal troops arrived in Galveston, Texas in 1865 to take control of the state and ensure that all enslaved people be freed. The troops’ arrival came a full two and a half years after the signing of the Emancipation Proclamation. Juneteenth honors the end of slavery in the United States and is considered the longest-running African American holiday. On June 17, 2021, it officially became a federal holiday. Juneteenth 2024 will occur on Wednesday, June 19.

June is Pride Month. Pride Month is an annual celebration of the many contributions made by the LGBTQ+ community to history, society, and cultures worldwide. In most places, Pride is celebrated throughout the month of June each year in commemoration of its roots in the Stonewall Riots of June 1969.
San Diego Pride will take place July 13-21. It will feature various events and celebrations including She Fest, Pride Parade, and Festival. The theme this year is "Making History Now".

Mama's Kitchen
Community Supports – Medically Tailored Meals/Medically Supportive Meals
Offers a medically tailored meal service through community support and delivers lunch and dinner to qualifying Medi-Cal members. Meals are provided for every day of the year, on a county-wide basis, free of charge. Mama’s Kitchen has partnered with Blue Shield and Kaiser to provide two nutritious meals a day for up to 12 weeks to qualifying Medi-Cal members. Scheduled meals rotate on a six-week menu cycle. The menu follows the Academy of Nutrition and Dietetics recommendations, and all meals are designed by registered dietitians and executive chefs. All meals are DASH and diabetic-friendly. Offers several menu variations to meet clients’ individual nutritional needs, such as low lactose, and vegetarian.

Emerging Models of Community-Led Care Infrastructure
June 24-26, 2024
Hahn University Center, University of San Diego
Join us for the 8th Annual CIE Summit! This hybrid event will be hosted in person at the University of San Diego campus’ Hahn University Center. This year will kick off with a pre-conference CIE 101 session on June 24th and two full days of engaging sessions on June 25-26.

The annual CIE Summit engages attendees from various sectors, including non-profit, health care, technology, government, public health, and philanthropy. The wealth of knowledge shared at the event is designed to benefit community-led care coordination efforts at each stage of development, including:
- System Build Phase or those designing or beginning to establish concepts for CIE or plan for CIE
- Launch Phase or communities that have recently deployed a CIE with a small group of partners
- Scale Phase or established CIEs focused on growing CIE network and infrastructure

Z Codes
Z-codes are the bridge between social health data and physical/mental health data and now partners can associate these standardized healthcare codes to Direct or Informational referrals.
By using Z-codes, partners can flag members as potentially eligible for programs and services. This can also support getting authorization to receive Enhanced Care Management and Community Supports services.

Appropriateness of Referral Notes
This year, we're focusing on improved referral quality and the need to understand why a Direct Referral didn't result in a connection to services. Based on common questions and feedback from partners, we updated the Referral Management process. Referral Notes are now required if someone marks a Direct Referral as not appropriate. This will help teams better identify referral quality trends and process improvement opportunities.

Updated Referral Detail Layout
The "detail view" of Referral records has an updated layout including information about the assigned case manager and clearer labels for notes fields. To ensure data quality, this layout is now Read-only.

CalAIM Screening
This new screening tool lists out the various eligibility criteria across all CalAIM programs, reviews qualifying criteria for each program type and helps the user navigate to the right type of program the client is eligible for. The screening is still being piloted but is open for feedback and training is forthcoming.

Recuperative Care Screening
This new screening tool was developed in partnership with the Hospital Association, Medi-Cal managed care plans, and San Diego County's local non-profit recuperative care providers - Father Joe's Villages, Interfaith Community Services, and PATH. The screening is currently in its piloting phase and can be completed only by specific CIE users.

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.
Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.
