⚕️ Community Health Assessment: What matters most to your health: mental health, housing, health care access or substance use? Share your opinion to help guide the next five years of local public health planning. Take the one-question poll. Every five years, local public health departments do a Community Health Assessment to see what areas are going well for community health and what could be improved. Because of your feedback from the last Community Health Assessment, behavioral health was identified as a community priority resulting in mental health and substance use public health programs and efforts.
