HCCI Selects Illinois House Call Project Cohort 2

HCCI is thrilled to announce the following selected practices for Cohort 2 of its Illinois House Call Project (ILHCP). The goal of the cohort is to collectively enroll and serve 3,000 new homebound patients by 2026, foster high-performing home-based primary care (HBPC) practices to improve patient outcomes, raise awareness of HBPC's value across Illinois, and gain insights into value-based payment opportunities for home-based medical care programs.

As part of the program, these practices will receive expertise, guidance, and comprehensive resources so they are well-positioned to lead and champion HBPC across the state. New Innovation Award Offers Free CCM Implementation Packages

The newly announced HCCI & Elea Institute Innovation Award program is intended to support Illinois-based palliative care organizations in implementing Chronic Care Management (CCM) services to expand and diversify their services and assist with driving revenue.

 

To achieve this goal, HCCI will select up to 10 community-based palliative care programs to participate, beginning in October 2024, from one or more of the following Illinois counties: Boone, Cook, DeKalb, DuPage, Kane, Kendall, Lake, LaSalle, Lee, McHenry, Ogle, Will, and Winnebago. Award recipients will receive a fully subsidized CCM Implementation Package.

New Course Licensing Program Launches

To better meet your needs and the needs of all of those in the home-centered care industry, HCCI is making some BIG changes. We’ve listened to your feedback and combined it with our own research findings to create several new initiatives designed to make it easier for you to work with HCCI and access the important information and resources you need. The changes are focused on the Four “C’s” — Contemporary, Community, Connection, and Competence. The first change, around “Competence,” came last week, with the launch of our new Course Licensing program.

Using Home-Based Primary Care to Improve Quality of Care & The Bottom Line

When Rush University Medical Center in Chicago decided to re-engineer its delivery systems to address population health, reduce health inequities, and combat chronic diseases while controlling healthcare costs, they connected with the Home Centered Care Institute. Together, they launched the innovative Rush@Home house call program.

 

Based on a six-month study, the Rush@Home program achieved the following:

• 34% potential lower cost

• 18% fewer hospitalizations

• 21% reduction in ED visits

• 4.6% fewer readmissions

 

Total Program Savings

• Total Actual Six-Month Savings: $761,000

• Total Potential Annualized Savings: Over $1.5 million

 

Based on these results, a program with 200 MSSP patients would generate over $3 million in savings.
