CCM Phase I Go Live Information


The Department of Health Care Policy & Financing's (HCPF) Office of Community Living (OCL) would like to thank everyone who attended the Care and Case Management (CCM) System Go Live - Roadmap to Success presentation on February 14. It is very exciting to be at this juncture in the project. 


Here is a brief recap of some of the information provided for those of you who were unable to attend or just want a refresher, including links below to a copy of the presentation slides and a Roadmap to Success guide.

Support Level Mismatch Reports for Community-Centered Boards


HCPF has implemented an update to the Bridge system. This update creates a requirement for a Case Management Agency supervisor to review all Home and Community-Based Service (HCBS) Developmental Disabilities (DD) waiver Prior Authorization Requests (PAR) with an effective date of March 1, 2023, forward. It is the responsibility of each case manager and supervisor authorizing the PAR to ensure the accuracy of the services being approved. This includes accurately completing Risk Factors, Support Level Override, Support Level History, and selecting the correct service line to match the member's calculated Support Level. The Support Level Verification process with screenshots can be found at this link


In addition, the system is being updated to implement an authorization limit of 366 units for Individual and Host Home Residential services in the HCBS-DD waiver.

About the Learning Collaborative Series

As HCPF navigates change and opportunity in the Colorado Home and Community-Based Services (HCBS) system, including Case Management Redesign, this monthly series of 50-minute discussions aim to provide individual Colorado HCBS case managers the opportunity to consider new strategies, gain knowledge, and enhance strengths – especially during times of transition. Each session will offer a live presentation, attendee activities, group discussions and time for questions and answers regarding the monthly topic. 


For those of you who joined us for prior case manager sessions in 2022, we heard you, and we have adapted to a shorter time (under an hour) and a new series of topics that allow participants to attend one, several, or all webinars depending upon your preferences.
