Greetings to all Case Management Agencies (CMAs)! Your input has been invaluable, and in response, we are excited to offer a series of Care and Case Management (CCM) system trainings to enhance your learning and mastery. All Case Managers are encouraged to attend.

Care and Case Management Support

To request support for a CCM system issue, case managers should call the CCM Support Center (888) 235-6944 or complete and send a CCM Support Request Form attached to an encrypted email to ccmsupport@assurecare.com. Support is also available on the FAQ and Known Issues pages.


For User Access to CCM and Bridge, please complete a 3rd Party System User Access Request Form, including the required signatures from your agency management. Submit the form to HCPF_OCLSystemApplications@state.co.us

Children's Buy In Program Stakeholder Engagement Opportunity

The Health First Colorado Buy-In Program for Children with Disabilities allows families of children with a disability who qualify to 'buy into' Health First Colorado (Colorado's Medicaid Program) for that child. If your child and/or your family earn too much for Health First Colorado or Child Health Plan Plus (CHP+) your child may qualify. If your child does qualify, you may pay a monthly premium to be in the program. We base your monthly premium on your family's income.


Buy-In members, stakeholders, and partners are invited to attend this meeting to learn about upcoming eligibility policy changes to the Children's Buy-In Program.

National Core Indicators–Intellectual and Developmental Disabilities (NCI-IDD) Survey 

HCPF is conducting the National Core Indicators–Intellectual and Developmental Disabilities (NCI-IDD) Survey to learn about the quality of life for those receiving services enrolled on the DD and SLS waivers until May 31, 2024. 


From now through May 31, interviewers from Vital Research will be contacting members to schedule interviews. Member participation is completely voluntary.


Members may have a proxy present during the survey. A proxy can be a family member, guardian, friend, or service provider. Case managers and service coordinators may not serve as a proxy. 


Members may choose to participate in person or over Zoom. The survey should take between 45 minutes to an hour to complete.

CMA/DVR Roundtables

The CMA/DVR Roundtable targeting Phase 3 CMAs which was scheduled for May 21, will be postponed to later in the year. In light of the current status of the Case Management Redesign and acknowledging the magnitude of the challenges currently facing the system, the Roundtable will not be held until after all CMA Transitions have occurred. Please watch for a new Zoom registration link for Phase 3 CMAs and DVR staff. 


DVR and HCPF Supported Employment staff will still be available from 10-11 a.m. on May 21, 2024 to answer any questions related to supported employment. The original Zoom registration link may be used to access this Open House style session, all Case Managers, DVR Counselors, and Providers are welcome to attend. Telligen Office Hours

Telligen is hosting office hours for case managers to answer questions regarding Utilization Review Utilization Management (URUM) reviews for: 

 

  • Health Maintenance Activities (HMA)
  • Over Cost Containment (OCC)
  • Supported Living Services (SLS) Exception 
  • Children's Home and Community-Based Services (CHCBS) 
  • Children's Extensive Services (CES)

Learning Collaborative Series for Colorado HCBS Case Managers

As we navigate change and opportunity in the Colorado HCBS system, including Case Management Redesign, this monthly series of 50-minute discussions aims to provide individual Colorado HCBS case managers the opportunity to consider new strategies, gain knowledge, and enhance strengths – especially during times of transition. Each session will offer a live presentation, attendee activities, group discussions, and time for questions and answers regarding the monthly topic. Please email any questions or concerns.

Compassion fatigue impacts a wide range of professions and caregivers. It is most common among professionals who work in a healing or helping capacity. People who listen to the stories of fear, pain and suffering of others may feel similar fear, pain and suffering because they care. Join us for a webinar where we will discuss how to define and identify compassion fatigue and steps you can take to mitigate the experience.

ARPA 5.04 Care-Case Management Best Practices Project Update 

HCPF has contracted with John Snow Research & Training Institute, Inc. (JSI) to conduct a stakeholder-informed process improvement initiative to improve communication, collaboration and coordination for members receiving long-term services and supports (LTSS) who are assigned to an RAE and a Case Management Agency (CMA). 


After identifying best practices to improve coordination, communication and collaboration between the RAEs, CMAs and HCPF, JSI outlined the roles, responsibilities and core competencies needed to implement and sustain the best practices. Over the past month, JSI has engaged CMA and RAE stakeholders through a survey to assess what type and level of training would be helpful to successfully implement and maintain the best practices.


Here are preliminary results from the survey:  

  • Number of Respondents: 14  
  • The preferred format for training: Live Webinar/Live Training  
  • Top four areas where respondents indicated training would be most helpful: 
  1. Documenting formal processes for communication/collaboration 
  2. Creating and executing memorandum of understanding (MOUs) and data agreements 
  3. Documenting standard workflows 
  4. Utilizing shared workspace 

 

Over the upcoming months, JSI will work to create training materials to support the implementation of the best practices as well as provide hands-on implementation support to the CMAs, RAEs and HCPF. Please watch for future updates on the project.
