Every month since February of 2020, former Care Ring patient James N. has sent in a note of thanks like this along with a donation. Because of our supporters, we were able to be there for James during a very dark time in his life: he had a debilitating health condition, could not work, did not have any income, and was going through the long and difficult process of applying for Social Security Disability benefits. He had no means to get health care, was in a precarious and unsafe living situation, and didn’t know where to turn for help. He felt hopeless and even had thoughts of suicide. We are so glad he found our Physicians Reach Out program, where our providers and our case managers could help him with his physical needs and get him connected to resources like housing and a phone. 
We took care of him until he was approved for disability and able to secure Medicaid coverage. James now “pays it forward” with his monthly donations, and his notes of thanks remind us of the incredible impact that our funders, donors and staff make for over 7,400 people each year. Thank you to the Charlotte Mecklenburg Community Foundation for their recent $30,000 gift in support of Nurse-Family Partnership. Established in 2001, Charlotte Mecklenburg Community Foundation is the primary grantmaking arm of Foundation For The Carolinas and is led by a local board of advisors.
Thank you to Corporate Champion Blue Cross Blue Shield of North Carolina who in addition to being a sponsor recently provided a grant of $50,000, which will support our Low-Cost Clinic operations during a critical time when we expect a shortfall in revenue.
YOUR IMPACT ON OUR 4 PROGRAMS:
Primary care for people with modest resources

Our Low-Cost Clinic has once again been recognized by the American Heart Association and the American Medical Association for its efforts and outcomes for patients living with diabetes and high blood pressure.

68% of our patients have their blood pressure under control, as compared to 50% of the American population. This is important because heart disease and stroke are the #1 and #5 causes of death, respectively. High blood pressure is a risk factor for both these conditions. READ MORE

74% of our diabetic patients have their A1C levels under fair control (<9%), with a goal of <7%. This is important for heart health because over time, high blood sugar can damage blood vessels and the nerves that control your heart. READ MORE

Comprehensive healthcare for those most in need

Congratulations to Physicians Reach Out provider Akshay Pendyal, MD on winning the Maribelle Connerat Award, named after our founder. He was nominated by Care Ring staff member Carla Gomez for his multi-year, high-level commitment to seeing our cardiology patients at the two Novant Health Heart & Vascular Institute locations where he practices.
And congratulations to Atrium Health Urology Kenilworth who won the Physicians Reach Out Practice of the Year Award, a new recognition, for their unwavering dedication to seeing our patients even in the face of significant challenges and transitions the practice was going through over the past year or so. Care Ring staff member Nabora McAvoy nominated them.
Personalized support for at-risk moms

We had 44 families graduate from the program for the first half of 2022, and they were celebrated with a Doctor Seuss themed event in August. Guests were treated to refreshments from Kona Ice and as usual the little ones looked adorable in their graduation caps and gowns! We commend the mothers for their commitment to completing the program and creating a brighter future for themselves and their children. Thank you to our NFP Nurses and our funders and donors for their dedication to these families!
Support for new and expecting parents

A Guided Journey (AGJ) recently turned one! Our funding partner, Mecklenburg County, has released a report highlighting the program's first year, which you can read HERE.

Care Ring's AGJ achievements include: 286 families served, 83% of clients have a primary care medical home, and 96% of women were screened for post-partum depression. Services provided included connection to critical resources like child care, nutritional assistance (SNAP or WIC benefits), pregnancy classes, medical care, and rent or utility bill assistance. Your support and trust made our first year a big success!

Each year we hold a heartwarming holiday event for the mothers and babies in our Nurse-Family Partnership program. To ensure participants' health and safety, the event has shifted to the outdoors, and is set up "trunk or treat" style, with families picking up needed items and toys as they visit each holiday-themed vehicle. We anticipate that this year's event will have 600 guests as we invite our new program families from A Guided Journey to join in the fun.
