April 23, 2021 - This week saw a catastrophe worsen in India, driving global numbers to stratospheric records, possibly to be dwarfed again in the coming days, as the Indian infections curve is still shooting straight up. Friday, India published the highest daily report released by any nation since the pandemic started, with 345,147 new cases. The ravaged country also reported a new fatality record of 2,621 single-day covid-19 deaths on Friday. Reports of struggling graveyards, overbooked crematoria, and a frantic search for hospital beds and oxygen is overwhelming the nation’s already stressed medical infrastructure. Even though India is the largest global producer of vaccines, according to Asia Times, at the current rate, “vaccinations for Indians will not be complete before November 2022”. This week alone, India added 2,050,773 new infections and 13,876 covid-19 deaths, respectively representing 36% and 16% of the global weekly total. Brazil is also in deep trouble, while other significant flares are showing up in Turkey, Cyprus, Greece, Argentina, Iran, Iraq (now ranked 25th globally), Kazakhstan, Cuba, Colombia, Guatemala, Peru, Holland, Canada, Chile, Cambodia, Kuwait, Mongolia, Tunisia, Venezuela, Pakistan, and Sweden. Japan is also showing active viral spread in a fourth national wave that threatens to match or exceed its previous January 2021 record-setting numbers.
Global deaths were up 7% with new cases up 10% since last Friday. 5,719,184 new covid-19 infections were confirmed worldwide, the highest weekly number yet, while the virus and its variants killed 87,132 people globally in the last seven days, the highest weekly global death toll since the first week of February.
Bloomberg reports that 1 billion doses have been administered in 172 countries, at the reported rate of 18.5 million doses a day.

As indicated in the pie charts below, the top five most infected countries account for half of the global cases and 44% of the deaths, while only representing 27% of the world’s population.

After a month-long upward trend, this week ended with a 12.6% reduction in new covid-19 infections and 2% fewer coronavirus deaths in the US. The nation still reported 437,611 new infections and 5,057 new deaths in the last seven days. The seven-day daily average of 66,515 still exceeds the first and second post waves averages. A few states that reported significant flare-ups seven days ago had much better results this week: Michigan -29%, California -27%, Pennsylvania -29%, New York -16%, Illinois -15%, Florida -7%, but a few other states confirmed higher numbers this week: Tennessee +26%, Washington +5%, Texas +2%, while most other states reported numbers close to those published last week. As of last Monday, anyone older than 16 can obtain a vaccine in the US and the CDC just lifted the two-week pause on the one-shot Johnson & Johnson vaccine, thereby adding 9 million doses to the national vaccination arsenal against the deadly disease. According to the CDC, to date, 83.2 million have received two doses and 137.2 million have received their first dose, for a total of 220.4 million doses administered. So far, 67% of US residents have received at least one shot. The wide availability and active delivery of vaccines nationwide might explain the sudden, and hopefully definitive, reversal of new infections numbers we have experienced this week. The near future will either confirm or deny this possibility.

This week, California's 15,370 new coronavirus infections total was significantly down from last week (-27%), as were fatalities (-25%), with 480 deaths counted since last Saturday in the entire state of California. Mass vaccinations in California are accelerating and now available to all aged 16 and older. 27 million Californians (44.1%) have received at least one dose.

The Madera County Department of Public Health COVID-19 Update:
4/23/2021: Reporting 10 new cases from the public bringing the total number of reported cases to 16,228.
Of the 16,228:
- 325 active case (including 3 Madera County residents hospitalized in Madera County)
- 15,663 recovered (1 released from isolation)
- 240 deceased

Including its prison population, over the last 7 days, Madera County is averaging 10.3 new cases per day (6.4 per 100K population), with 72 new cases revealed over the last 7 days (-55 week-to-week), and 8.9 new cases per 100K over the last 14 days, confirming the county's red tier status.
Today, the seven local counties together reported 176 new infections and 10 new coronavirus deaths. In the combined 7 counties, COVID-19 has infected 329,430 people and has killed 4,781 residents of our region since it claimed its first central valley victim, in Madera, on March 26, 2020.
Vaccinations are now available in Madera County for anyone above 16 years of age. The Madera County Public Health Department is also making the shots available without appointments in drive-through vaccination clinics. Visit the MCPHD Facebook page for details.

For the last seven days, new infections totals reported in the seven local counties were down 30% week-to-week, with 1,246 confirmed new infections, the lowest numbers reported since the end of May 2020. This week reported 49 new fatalities, 6 more deaths than were confirmed the previous week.

Keep observing the simple yet proven safety habits of physical-distancing, mask-wearing, and frequent hand-washing, that will help drive down new infections and new deaths, to a level low enough so as to give us a chance to reopen our schools for onsite education and thus, reopen our economy. Nothing else will work until we have massively administered the freely available safe and effective covid-19 vaccines -- currently estimated by vaccinating up to 85% of the population -- to finally reach herd immunity. Anyone over 16 can get vaccinated in the US as of last week.
