Dear CHNA 6 Member, 

Welcome to our monthly CHNA 6 Newsletter! We are hoping that this newsletter will provide our dedicated members with information to better connect you to our growing network. 
CHNA 6 Updates:

This year CHNA awarded approximately $417,000 in large and mini-grants to 17 non- profit organizations and agencies throughout  communities in Central MA.  The grants were awarded for the first year of DoN funds (Determination  of Need) in a process guided by the Department of Public Health to support local health initiatives. DoN funds were available as a result of the construction of new Emergency Department, ICU and private patient rooms at Milford Regional Medical Center.  The grant projects focus on access to care, behavioral health, interpersonal violence prevention, and chronic disease prevention.  These areas were identified by the 2015 Greater Milford Community Needs Assessment and CHNA 6 developed a Community Health Improvement Plan (CHIP) to address these areas.
 
Over 50 agencies participated in developing the CHIP targeting the 13 communities served by CHNA 6. Work groups met for over a year to develop the goals, objectives, and strategies of the CHIP and we will be reconvening to review the CHIP this November.  We hope you will join us for upcoming meetings (see below) as we learn more about current projects, and plan for Year 2 of funding in early 2017.

Fall Meeting Dates:

CHNA 6 Meetings will be held next Fall at the following dates and locations, from 9-11 AM. Our upcoming meetings are:
  • Friday, September 16, 2016: Ruth Anne Bleakney Senior Center, 60 N. Bow St. Milford
  • Friday, November 4, 2016: CHIP Summit at Milford Regional Medical Center, 14 Prospect St. Milford, Conference rooms A & B

FYI: Attendance is required at 50% of meetings for those receiving CHNA 6 grants
