IMPORTANT MESSAGE REGARDING POTENTIAL CHANGES TO ANOTHER HEALTH INSURANCE CARRIER, BENEFITS, AND PROVIDERS

You should soon receive your annual Pre-Tax Bi-Weekly Election Form regarding your Health Benefit Elections. Check the date on the Pre-Tax Bi-Weekly Election Form. The date on the form should reflect the date that you sign it. If it does not, please bring it to the attention of your Labor Representative, Ellena Osborne before signing.


Make a copy for your records. Immediately, send that copy to JNESO!

ARTICLE 24 – HEALTH BENEFITS

(Pages 28-29 of your Union Contract)

The Terms for the Two Open Enrollment Periods:


1. Health Benefits Plan:

a. Benefits changes/coverage will be effective annually.


b. Bishop will provide members with:

i. A summary of the plan’s description insert/booklet, a summary of benefits/coverage


ii. A form to maintain, change or waive the health benefits


After receiving this information, members will have thirty (30) days to review and sign this form.


2. Premium Rage change:


c. Members will be notified thirty days before any rate changes.


d. All acceptances, revisions, or waiving of the plan must be done in writing.


3. Bishop will notify the JNESO office by providing the documentation showing any Benefits/Coverage or Premium Rate Changes.
