10 Years of IDDSI - Why Temperature Matters

Temperature is important and we know many of our global community are currently experiencing hot temperatures while other parts of the world are much cooler.
|
|
Temperature is also important to consider when preparing and serving drinks for those who experience swallowing difficulties.
This is because thick liquids change thickness levels depending on their temperature. It has always been the case that the thickness of liquids change depending on their temperature. Generally speaking, a thick liquid that is served cold/chilled will be thicker than if it is served at room temperature, or heated. We see this effect in our daily lives as well. Think about a tube of toothpaste. During winter, more force is needed to push the toothpaste out of the tube, as the toothpaste is firmer. During the summer months, it is very easy to push the toothpaste out of the tube as it flows more easily due to the ambient heat.

Think now also of a rich pumpkin soup. Served warm it behaves more like a liquid, but when it cools off, it becomes thicker. This is physics at work. Thick liquids have always behaved this way – thicker when cool, and thinner when warm. In research this is why it is critical to include the temperature for testing because the effect is well known.

How does this affect individuals with swallowing difficulties? Thickened milk is typically served warm to babies. The teat/nipple needs to be carefully selected so that the thick milk will flow through it when the infant sucks. If the baby is fussy, the thick milk cools over time and the thick milk becomes thicker and harder for the infant to suck through the teat/nipple. This can cause the baby to fatigue more quickly as they become inefficient with their feeding. We also know that for some individuals with swallowing difficulties, thicker consistencies may require more effort to manipulate in the mouth and can leave residue in the throat after the swallow.
How can I check the thickness level?
The IDDSI Flow Test, (and for Level 3 and 4) the Fork Drip test and the Spoon Tilt Test can be used to accurately capture the change in thickness level as the fluid cools. It is important to always check the thickness level at the point of serving.

The International Dysphagia Diet Standardisation Initiative (IDDSI) and the Canada IDDSI Reference Group are delighted to announce that we are hosting our first annual Canadian IDDSI Festival on Thursday, October 26, 2023, at Humber College in Toronto, ON. This event is an ideal opportunity to network, discuss best practices and learn about innovations to support IDDSI implementation in Canada. The event is open to everyone who supports and/or implements the IDDSI framework.
The Canada IDDSI Festival will feature Dr. Catriona Steele, ASHA Fellow, Canada Research Chair in Swallowing and Oral Food Processing and Marianne Katusin, Director of Support Services, Halton Healthcare as our keynote presenters with an opening welcome & IDDSI global update from Peter Lam, IDDSI Chairperson & CEO. There will also be a panel discussion to address IDDSI challenges, innovations and solutions. Participate in practical demos and hands-on learning workshops. Visit with vendors and discuss with them your IDDSI wish lists. The event aims to inspire delegates to put theory into practice to support residents and patients with dysphagia. IRG IDDSI Spain was born to facilitate the access of people with dysphagia to food in their community. This group aims to adapt the IDDSI framework to the reality of Spanish gastronomy, as well as being a point of support for professionals, family members and people with dysphagia. No person with dysphagia without their food, no caregiver with doubts, no professional with fear. We are here to help and work as a team!

Just getting off the ground is the Paraguay IRG in Latinamerica. The group is composed of speech pathologists (phonoaudiologists), nutritionists and physicians. At present they have begun to meet to discuss ways to introduce IDDSI in different settings and also ways to begin implementation. The initial focus is in Asunción, the nation’s capital.
