Illinois community-based palliative care programs can receive complimentary Chronic Care Management (CCM) implementation support from the Home Centered Care Institute (HCCI) by applying for the new HCCI and Elea Institute Innovation Award. By adding CCM, programs can broaden their care options, reach new patients, and boost revenue.

HCCI will choose up to 10 community-based palliative care programs in Boone, Cook, DeKalb, DuPage, Kane, Kendall, Lake, LaSalle, Lee, McHenry, Ogle, Will, and Winnebago counties to participate, beginning in October. Award recipients will receive a fully subsidized CCM Implementation Package. Apply here by August 30.
