The desire for independent and healthy living is now easier than ever for seniors in and around Bardstown with Care Guide Partners PACE.
As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people aged 55 and older with the support they need to stay in the homes they know and love as they age.
We proudly serve residents of Bullitt, Grayson, Hardin, Hart, LaRue, and Nelson counties.
From its new PACE Center in Elizabethtown, Care Guide Partners coordinates and delivers all the medical care, senior services, transportation, and in-home support participants need to maintain their independent lifestyles. The program truly is a “one-stop shop” for seniors who need support to age safely and comfortably at home. It’s also a big help to those who love them, with:
- 24/7 comprehensive health and medical care including primary and specialist services
- Transportation to and from the PACE Center and all program-coordinated appointments
- Outpatient labs and X-rays
- Dental, podiatry, optometry, audiology care and medical supplies
- Physical, occupational, and speech rehabilitation services and equipment
- Meals, nutrition planning, and nutritional support as needed
- An adult day program at the PACE Center that offers meals and social activities like day trips, parties, and cultural celebrations
- Other support services, including laundry and personal hygiene assistance
Customized Care Plans
At PACE’s heart is a team of geriatric medical specialists and experts on aging who put together a custom plan with each participant. From there, Care Guide Partners PACE coordinates everything, from scheduling medical appointments to providing transportation, social services, meals, and any other day-to-day assistance required based on your unique needs.
Who qualifies for PACE?
To be eligible for PACE, seniors must:
- Be at least 55 years old
- Be certified by the Commonwealth of Kentucky to need nursing home-level care
- Be able to live safely in the community at the time of PACE enrollment
- Be living in Bullitt, Grayson, Hardin, Hart, LaRue, or Nelson counties
How much does PACE cost?
PACE is free to participants with Medicare and Medicaid. People with Medicare-only or private insurance should contact Care Guide Partners to discuss eligibility and copay possibilities.
