Effective Authorization of Community Connector and Homemaker services in the HCBS-CES and HCBS-CHRP waivers
HCPF is providing resources and training to case managers on effectively authorizing Community Connector and Homemaker services under the HCBS-Children's Extensive Services (CES) and HCBS-Children's Habilitation Residential Program (CHRP) waivers. The Department would like to ensure Case Managers are utilizing best practices during the eligibility and service planning process including age-appropriate guidelines.
Upcoming Required Training Course and Office Hours
HCPF is launching a required training for all case managers and case management supervisors who work with members on the HCBS-CES and HCBS-CHRP waivers.
HCPF will release the required training course in the Case Management Learning Management System titled Effective Authorization of Community Connector and Homemaker Services in the HCBS-CES and HCBS-CHRP waivers on September 20, 2024. A supporting user guide will also be made available.

Case managers are required to complete the course by September 30th and are required to attend the Zoom Office Hours on October 1st from 1 to 2 p.m. where the information will be reviewed and allow an opportunity for case managers to ask questions.
