APP SPOTLIGHT

Roxanna Benavides, NP and Neil Wiegand, PA-C Goes Global


Roxanna Benavides, NP, General Surgery (in photo: left) and APP manager Neil Wiegand, PA-C, both members of the UCSF Center for Global Health Nursing, traveled on 8-day Global Health trip to Nuevo Progresso, Guatemala. Their US-based team of two general surgeons, a general surgery resident, a scrub RN, a scrub tech and Roxy and Neil provided free surgical care to approximately eight adult patients per day at the Hospital de La Familia. Case types included hernia repairs and removal of masses. Pediatric patients were also treated for a total of 50 patients. Roxy served as an integral part of the Team as a fluent Spanish speaker conducting the histories and physicals. She nearly single handedly oversaw morning and evening rounds on the patients and scrubbed into the OR and assisted with some General Surgery cases. Roxy was the first NP on this Global Health team and proved the value of APPs over-all in the success of this trip and future trips. Alice Parker, NP Pediatric Cardiac ICU Retires 33 Years of Service


Alice Parker, NP at UCSF Benioff Children's Hospital, is retiring after thirty-three years of distinguished service.

She began her journey at UCSF Medical Center in 1991 as a new graduate registered nurse on the 8 Long Neurosurgery and Neurology unit. In 1996 she started to work in the Pediatric Critical Care and knew that she had found her home and passion. She quickly became a clinical nurse III and updated standardized policies and procedures for Neonatal/Pediatrics and completed the educational and clinical standardization of the new graduate training program for the PCICU. With the desire to provide higher level of care to patients specifically with complex congenital heart disease and defects, she pursed a Masters degree at UCSF and graduated in 2015. As an APP, she joined the newly opened UCSF Benioff Children's Hospital at Mission Bay in the Fetal Cardiovascular Program in the Fetal Treatment Center involved in fetal research and the care of antepartum mothers with fetuses with critical CHD. Since 2020, she returned to her long-time “home” as a frontline Provider in the Pediatric Cardiac Intensive Care Unit. As orthopedic challenges have forced Alice to take early retirement, she looks forward to new adventures with her husband and two adult children. Her legacy at UCSF will be remembered through the countless lives she has touched and the many healthcare professionals she has mentored. Alice's remarkable journey reflects her commitment to excellence in patient care and her lasting impact on the UCSF community.

Improving Discharge Coordination in the Intensive Care Nursery: Implementing Discharge Roadmaps and Weekly Huddles

Chrissy Smith, NNP started a pivotal quality improvement initiative several years ago. It has developed into a Complex Discharge multidisciplinary taskforce with important impacts on our complex infants' discharge from the Intensive Care Nursery. The team's work was presented at the recent 2024 UCSF Health Improvement Symposium. This is just one tangible demonstration of how Chrissy and her team of superstars are improving patient care. Dana Jakoubek, NP Contributes to the 1st Allied Health Workshop International Liver Transplant Society Congress


Liver Transplant Department Dana Jakoubeck, NP (in photo: sixth from left), was the driving force behind the International Liver Transplant Society (ILTS) Allied Health Committee. As a founding member, she played a pivotal role in shaping the committee's vision and objectives. Starting with a group of three, Dana and her colleagues expanded the Allied Health Committee representation to twelve members from eight different countries. This year, Dana served on the planning committee for the first-ever Allied Health workshop at the annual ILTS Congress, a milestone event for the committee. Their efforts culminated in the organization's pre-meeting workshop and an Allied Health Professional (AHP) abstract session, providing a platform for sharing knowledge among international AHPs.

APP Blue Ribbon Award Projects


The UCSF Center for Healthcare Value (CHV) designated 10 projects to receive "Blue Ribbon Award for High-Value Care" at the May 31, 2024 8th Annual UCSF Health Improvement Symposium. The award targets projects geared toward reducing costs, improving value and reducing waste. Projects 1-6 listed below involved APPs. Projects 5 and 6 listed below were solely done and focused on APPs.

  2. Decreasing Insulin Pen Waste in Adult inpatient areas
  3. Reducing Unnecessary Radiation Exposure from Chest X-rays in Cardio-Thoracic Patients

  9. Answering the Call: Specialty Pharmacy’s Path to Sustainability
  10. Early Re-conversion of Nebulized Racepinephrine During National Drug Shortage Resulted in Improved Access to Medication

Christina Louie, FNP is Express Care


Christina Louie, FNP is delivering Primary Care at Laurel Village in an innovative way that is improving patient access for time-sensitive concerns. Together with Adrian Tomas, MD Clinical Informatics Fellow PGY-5 UCSF Division of Clinical Informatics and Digital Transformation (DOC-IT) and staff physician in the UCSF Department of Family and Community Medicine, Virtual Express Care was developed as a tool in MyChart. It allows patients to initiate a synchronous video visit with a provider on-demand for specified reasons such as vomiting, muscle/joint pain, diarrhea, cough/cold/flu, fever or rash. Data has shown that the patients typically wait 10-20 minutes and their Press Ganey Satisfaction scores are high. As a provider, Christina says "Express Care is better than all the back and forth MyChart messages with the patient". This pilot project has proven to be successful in impacting overall encounter volume, inbasket messaging volume, and patient satisfaction. This new care model will be expanding to include other UCSF Primary Care locations.

UCSF APPs Advancing Health Policy

Annika Ehrlich, DNP, FNP, is a nurse practitioner in the Headache Center and a national leader in headache advocacy. She was recently appointed president of the board of directors for the Alliance for Headache Disorders Advocacy– the first nurse to serve in this role. Read more in Science of Caring.

Rubi Alva de Hickson, CCRN, ACNP, is a nurse practitioner in Critical Care never expected to be involved in health policy work — let alone lead it. Currently in UCSF SON Post-Masters entry to DNP program, she is equipping nurses to be bold advocates; read more in Science of Caring.

STANDARDS & COMPLIANCE

.APPNOTE went live on May 14


There are improvements to the .APPNOTE smartform that will be going live on May 14, 2024 for the ED, inpatient and ambulatory settings. The new workflow aims to resolve compliance and efficiency challenges.

  • Time Spent and medical decision making statements are now built into the smartform. You no longer need to add this to your note separately.
  • Attestation of student documentation is now possible. When editing a note authored by an APP student or medical student, an attestation statement will automatically populate in the smartform.
  • By policy, UCSF APPs must document all clinical encounters in which they take part, even in a limited fashion. APPs are required to use .APPNOTE and indicate either independent or shared anytime they contribute to documentation (even for minor documentation).
  • Shared visits are defined as both the APP and physician making a significant contribution to patient care, both providers are from the same group, and provide care on the same day and the physician meets that bar for substantive portion
  • Either >50% of the total patient care time OR performed a substantive portion of the medical decision making, which is defined as making or approving the management plan for the number and complexity of problems addressed AND taking responsibility for the risks of complications and/or morbidity or mortality of patient management
  • Independent visits 100% of wRVU are attributed to the APP for shared visits 40% of wRVU are attributed to the APP and 60% to the physician.

APPs are wanted to join the Sexual Assault Forensic Examiners (SAFE) team at UCSF Children's Hospital Oakland. It is an ON-CALL position only and is a great addition to your current full or part time position (must be employed by UCSF Childrens Oakland).

The SAFE team is a 24/7 service that provides acute forensic sexual assault exams to children in the UCSF CHO Emergency Department. This is truly an incredible service our hospital provides and it's an amazing opportunity to provide specialized care to our patients and families. Our medical team would provide all the training you need to get started and we are always available to support all members of our on-call team in real time. Please note that at this time we can only hire APPs that works full time or part time at UCSF CHO and are employed by CHO. (at the moment we can't hire APPs outside of CHO). Create an Incident Report within APEX


You can now launch the RLDatix Incident/Event Reporting System directly from APeX to create an incident/event report on a specific patient. Patient information will automatically populate in the incident/event report, reducing the amount of manual data entry into RLDatix. Information from the incident/event report will not be saved in the patient’s medical record. Reminder: do not reference the incident report number or document the submission of an event report in the patient’s chart.

American Society of Clinical Oncology just released "Let Us Have the Conversation: Serious Illness Communication in Oncology: Definitions, Barriers, and Successful Approaches". Its 11 pages and available for free here. It is a nice summary of serious illness communication barriers and strategies for integrating good communication into routine cancer care.


2. Consider downloading Vital Talk app to your smartphone. It’s free wherever you get your apps. Contains simple talking maps --responding to emotions, delivering serious news, early and late advance care planning, etc. It’s a great reminder of all the important parts of serious illness communication
