APP SPOTLIGHT

Adult Critical Care APP Group Celebrates 20 Years

This year the Critical Care APP (formerly CCNP) group celebrates 20 years! In 2004, the 3-person NP group started in the 9ICU. Now there are more than 40 APPs (both NPs and PAs) that cover 7 different ICUs, 95 ICU beds, across 2 hospitals. Amongst the 40 APPs includes 7 NPIII, 6 APP Leads, 2 LeAP Academy graduates, the first APP EBP Fellow, the first Informatics champion, and a graduate of the VA Simulation Fellowship. Many team members are also in pursuit of their DNP. The Critical Care APP group remains committed to the education of the next generation as evidenced by prior manager Thomas Farley creating the first APP fellowship at UCSF-- Surgical Critical Care NP Fellowship (now APP Fellowship). Many APPs have passed through the Critical Care APP group as member, fellow or student before landing in other clinical settings. The group honors one of their best, Mellyn Reading, who succumb to breast cancer in 2017. A special thanks to those in leadership roles that have supported APPs working to the top of their license: Physician colleagues Michael Gropper MD for advocating for the group in the early days, continued support with Matt Aldrich, MD, and the current support of Kevin Thornton MD. We also recognize the APP Leadership that has supported the team throughout the years: Geoff Latham, Thomas Farley, and most recently Erika Wilson and Keith Jabonillo. Congratulations!!!

New APP-led clinic with Patricia Zendejas, NP

Patricia Zendejas, NP co-founded the Cancer Diagnostics Service (CDS). The CDS is an APP-led clinic aiming to expedite the diagnostic process for patients with a new suspected cancer diagnosis. Delays in the cancer diagnosis process can be associated with meaningful impacts in patient outcomes. The CDS aims to evaluate patients within 3-5 business days of referral, with an overarching goal to swiftly connect patients with definitive cancer care. Patty serves as the primary clinician at CDS, which has already received approximately 20 referrals. In the upcoming months, CDS plans to expand its reach by accepting direct referrals from primary care and other outpatient clinics at UCSF. Patty has played a pivotal role in establishing and expanding this vital clinic.

Sarah Donahue, NP Publishes in JADPRO

Sarah Donahue, MPH, NP, was lead author for a publication in the Journal of the Advanced Practitioner in Oncology (JADPRO), July/August Edition entitled, "A Real-World Evidence Primer for Advanced Practice Providers: Integrating P-Reality X Into Shared Decision-Making for People With HR+/HER2− Metastatic Breast Cancer". Advanced Lung Disease Team (ALD) Improves Discharge Before Noon from 12% to 39%

The ALD medical team focused on improving their DCBN rate to meet at least the UCSF Health goal of 20%. Over the course of one month, not only did they meet the UCSF Health goal, but they exceeded it by 19%. Their tactics were the following and involved the entire team: Notify the patient and their family 1-2 days prior to discharge and expressly tell them discharge will be in the morning; discuss potential discharges on PM rounds to ensure the team is on the same page. Identify barriers for discharge including delivery of medications, final clinical decision/diagnostics and family/transportation issues; discontinue telemetry/continuous pulse oximetry on pre-rounds, notify the bedside RN and inform the patient to get dressed and packed; in addition to bi-weekly MDR rounds, the APP, Pharmacist and Case Manager reviews all the medications and DME needs and orders placed the day before discharge; all patients being discharged are rounded on first in the morning or after pending labs have resulted. These simple tactics resulted in a DCBN rate of 39%. While their patients are far from simple, their tactics are universal and can be replicated by other care teams. Kudos to the entire ALD Medical team for their outstanding achievement.

Updated NP and PA Core Privileges


The NP and PA Core Privileges have been updated and are on the Office of Medical Affairs and Governance (OMAG) website: https://medicalaffairs.ucsf.edu/privileges

 

Review these core privileges and reach out to advancedpractice@ucsf.edu if you have any questions. Please note, all APPs must complete e-Logs in UC Me every 2-year reappointment cycle for core privileges and granted standardized procedures privileges. For core privileges, the UC Me e-Logs may be of any provider-level direct patient care activity that you performed and documented in APeX. Examples include new patient evaluations, progress notes, consult notes, procedure notes, and discharge summaries. The e-Logs submitted must correspond to documentation in APeX by the APP in order to pass audit.

Medical Incapacity Hold Policy


The purpose of the Medical Incapacity Hold policy is to delineate the steps to be taken when a patient without Capacity to Make Healthcare Decisions ("Capacity") is at serious risk of harm, disability or death, attempts or threatens to leave UCSF Medical Center without the provider's approval. Nurse-Provider Council Seeks APP Involvement


The Nurse Provider Council is led by APP Elisabeth Gunderson and RN Marissa Fung. The council was established to address issues that contribute to: improving patient care, enhancing collaboration between providers and nurses and strengthen communication across the practices. The APP JEI Committee is seeking APP volunteers for its Collaborative Career Advancement Network (C-CAN). APP volunteers are needed to provide shadowing experiences, personal statement review, mock interviews, and mentorship to employees interested in or pursuing a career as an APP. UCSF School of Nursing Moves Out of Parnassus During Demolition


The UCSF School of Nursing building opened in 1972. The 52 year old Parnassus Heights Building was the home to many nursing programs including masters entry, Masters in Science in Nursing and doctorate. The school's operations will temporarily relocate to the UCSF Wayne and Gladys Valley Center for Vision at the Mission Bay Campus. They will stay there until a new nine-floor research and academic building opens at the Parnassus Height campus that also includes the UCSF Health Helen Diller Hospital. Read more.

Bayfront Medical Building Opening at Mission Bay


The UCSF Bayfront Medical Building is a convenient adult outpatient center for scheduled medical appointment as well as same-day (urgent) care. Call for Proposals for 2024-2025 Clinical Nursing Research (CNR) Grant


This grant provides funding support for pilot research projects led by a collaborative investigator team that includes UCSF Health clinical nurses and UCSF School of Nursing faculty investigators. PAs may apply but team must include at least clinical nurses, NP, CRNA, midwife or CNS whose FTE is at least 50%. Office of Advanced Practice Providers (OAPP) Leader Rounds


At the end of July 2024, the OAPP leadership team of Ivette Becerra-Ortiz, Alisa Yee, Brandon Sessler, Roseanne Krauter and Tara Valcarcel started to conduct in person leader rounds in various clinical departments (inpatient and outpatient, pediatric and adults). The purpose of meeting you in person is to understand how you feel about your work, understand how you spend your work time and ask if its meaningful to you, and understand if your strengths and skills are/are not being fully recognized.
